Fond du Lac County is Hiring a Psychiatric Nurse Practitioner Near Fond Du Lac, WI

Under the direction of the Director and Medical Director, this position performs specialized duties in clinical settings, performs mental health assessments, manages the case of patients within the field of psychiatric mental health, and provides health education to those patients, specific to the needs of their mental illness.
 
 Position eligible for a $5,000 Sign On Bonus.Responsibilities:
  • Provides mental health assessments in a multidisciplinary team environment dealing with a wide range of mental illnesses. 
  • Provides assessments, crisis response, medication and medication injections, direct mental health services, and medical record documentation as required.
  • Formulates an initial diagnosis and treatment plan and monitors changes of clients with persistent mental illness.
  • Work collaboratively with psychiatrists, nurse prescriber, therapists, and case managers to ensure clients are receiving accurate medications in a timely manner and assure continuity of care, prevent overlapping orders, and to coordinate care as appropriate. 
  • Communicate written or verbal medication orders to the pharmacy. Check the status of current prescriptions. Refill existing orders per agency policy. Field and respond to refill requests from clients. Obtain medication prior authorizations from insurance companies. 
  • Review medication information with clients to ensure they have an understanding of the purpose as well as the risks associated with ordered medications. 
  • Serve as a resource for health and medication information to other staff members. Provide health education information to staff and/or clients when appropriate. 
  • Ensures professional psychiatric care for clients within DCP.
  • Ensures strict confidentiality of psychiatric treatment services per department guidelines and state administrative code.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Graduate of a Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.
  • The employee shall meet the requirements of the Wisconsin Board of Nursing; and be licensed to practice as an Advanced Practice Nurse Prescriber (APNP) without restrictions within the State of Wisconsin.
  • Experience working with persons diagnosed with broad range of psychiatric illnesses.
  • Ability to work in a team environment.
  • Ability to effectively communicate with varied and diverse staff and clientele. 
 
PHYSICAL REQUIREMENTS:
Ability to freely move in the community (such as through possession of a valid Wisconsin Drivers License, or other means) including access to a vehicle.
SPECIAL REQUIREMENTS:
Ability to work a flexible schedule when workload dictates.
Candidates with experience working with children preferred.
