What Will you be Doing?
  • Provides patient care as directed on aide care plan, reports and documents observations and care performed.
  • Observes and reports changes in client’s condition to Supervising Clinician.
  • Assists the patient in ADL’s (Activities of Daily Living) as directed on aide care plan.
  • Does not discuss patient or patient’s information outside of the Agency – maintains confidentiality
  • Communicates patient’s supply needs to Supervising Clinician or Health Care team.
  • Displays professional behavior at all times.
  • Complies with all applicable local, state and federal regulations.
  • Complies with all areas of corporate compliance program.
  • Documents thoroughly and accurately in format prescribed by the agency.
  • Attends In-Service Training programs to maintain currency in practice and certification.
  • Must comply with HIPPA regulations as they pertain to PHI and assorted patient information.
  • Perform related duties as assigned.

Environmental Conditions and Physical Requirements
  • Positions in this class are typically situated in a home environment. This position may be required to go out into the field alone to perform patient care. As a result there will be exposure to all types of weather, from snow to rain to temperatures in excess of 100° Fahrenheit. The ability to gain ingress and egress at all types of locations (e.g. homes, apartments, mobile homes and other domiciles is essential.)
  • Position routinely performs cooking, transfers, toileting and bathing. These actions require the mobility to lift patients, stoop, bend and reach.
  • Driving in a motor vehicle, standing, sitting, walking, bending, reaching and stretching.

Oakland
Burlingame
San Rafael/Novato
Minimum Requirements:
  • Able to provide two verifiable references.
  • Must have vehicle, current driver’s license, and appropriate automobile insurance coverage.
  • Minimum age of 21.
  • Minimum 1-year experience as a Home Health Aide.
  • High School Graduate with valid diploma.
  • Current certification as a Certified Home Health Aide by the State of California required.
  • Current CPR card.
  • Documentation to verify work experience and legal right to work in the U.S.
  • Reports to work in a timely manner and adheres to call-in times for schedules changes.
  • Regularly participates in case conference, in-service training and corporate compliance programs, as required.
