Qualifications
JOB SUMMARY
The Reentry Facilitator / Care Manager is a hybrid teaching and counseling position that provides support to clients who are referred by Alameda Probation; builds and maintains successful working relationships with a network of local service providers and probation officers; conducts pre-release screenings; tracks progress through evidence-based Cognitive Behavioral Intervention classes; and documents individualized case notes and program outcomes.

JOB DUTIES AND RESPONSIBILITIES
- Work directly with probationers, the Alameda Probation and Sheriff’s Departments, and local service providers to plan for reintegration from incarceration into the community
- Conduct initial pre-release screening to determine the level of service and support needed regarding education, employment, family, recovery, and community
- Complete a certification in CBI-Employment and Motivational Interviewing upon hire
- Develop individualized reentry and stabilization plans with a caseload of clients
- Facilitate daily workshops for probationers in CBI-Employment, a ten-week cohort model program
- Develop, revise, and implement curriculum that aligns with the evidence-based practices of the CBI program
- Develop and facilitate strategy for community education and awareness of the needs of formerly incarcerated clients
- Network with other reentry programs to referrals to other services as needed
- Make referrals to probation department employment program as needed
- Keep accurate and up-to-date records and information on clients, employers, placements, and all related data in accordance with standards utilizing Apricot 360 or other client databases
- Report progress, outcomes, and activities on an on-going basis
- Conduct program intake and exit interviews
- Monitor and evaluate progress of participants in carrying out Personal Responsibility Plan
- Document participants progress and maintains appropriate documentation in case files
- Assist with program orientation as needed
- Coordinate efforts of internal and external partners to prove case management services to participants
- Provide counseling and crisis intervention services to program participants
- Take a lead role in criminal justice and reentry training, planning and implementation initiatives as needed
- Other duties as assigned
KNOWLEDGE AND SKILLS
- Excellent public relations, community organizing, negotiation, mediation, conflict management, and meeting facilitation skills
- Computer literacy in word processing, email, internet, and spreadsheets
- Must have strong administrative and organizational skills including budget development and management, and grant administration and reporting
EDUCATION AND WORK EXPERIENCE
- BA degree (MA degree highly preferred) in Psychology, Sociology, Social Work, Counseling or a closely related field is preferred
- 2 years of case management, counseling, and assessment experience
- Previous reentry experience preferred OR Equivalent combination of education and experience
PHYSICAL REQUIREMENTS
- Must wear provided PPE (Personal Protective Equipment) and adhere to all safety guidelines and requirements, when needed
- Frequent talking and hearing conversations
- Frequent and prolonged standing, walking, and sitting
- Long periods of walking, sitting at a desk and/or computer station
- Lifting items that weigh up to 25 lbs. or more; reaching, bending, carrying, and stooping within the daily routine of tasks and activities
ADDITIONAL REQUIREMENTS
This position is contingent upon:
- Satisfactory results of a Live Scan (criminal background investigation)
- Successfully obtaining jail clearance for positions working in county jails or probation locations
- Submitting proof of clear TB results. Results dated within 60 days prior to hire date are acceptable
WORK ENVIRONMENT
- Must be willing and able to work in a correctional facility and travel throughout the area to community sites
- Work environment varies from site to site
- There is some personal safety risk working with the at-risk population
- Moderate noise level
- Travel to multiple sites required
