POSITION PURPOSE:
Performs evaluations, planning, and implementation of occupational therapy treatments for inpatients and outpatients. Assist with department goal setting, program planning, clinical instructions and staff instruction in order to maintain a progressive and educationally motivated staff.
QUALIFICATIONS:
Graduate of an approved program for occupational therapy with a Bachelors degree or higher
National Board of Certification in Occupational Therapy registered or eligible to take exam
Current/Active license to practice in the State of Nebraska
Valid Nebraska Drivers License with good driving record
Must have reliable vehicle
Basic Life Support (BLS)
Advanced Cardiac Life Support (ACLS) preferred

Will have access to confidential information abiding by the organizations privacy policies and regulations concerning this information.
Essential Job Duties and Responsibilities
1. Evaluates, plans and administers treatment to patients in accordance with physician orders, and accepted standards of professional practice.
Provides individual treatments for activities of daily living (ADL).
Evaluates patient treatment goals for needed changes according to patient condition.
Develops treatment plan in collaboration with patient, family support system and members of health care team.
Completes treatment plan to meet regulatory guidelines.
Incorporates plans for discharge in treatment plan in collaboration with patient and support system.
2. Evaluates treatment efficacy and patient progress and makes necessary adjustments in the care plan to optimize patient care.
Educates family and/or support system to functional needs, equipment/resources to meet these needs.
Assures the safety and efficacy of equipment used to provide rehab services.
Documents assessment, treatment goals, and plan of care according to regulatory guidelines.
3. Maintains complete and accurate records pertinent to patient treatment and progress to assure proper documentation and to be in regulatory compliance.
Attends and participates in family conferences, patient care conferences, medical rounds and as required by areas of services to establish goals, formulate discharge summaries and follow-up plans, including home programs.
Participate in setting and meeting departmental goals by participation in performance improvement projects, on-going program planning and implementation.
4. Maintains effective communication with Manager, Lead, Director and other patient caregivers regarding patient care decisions.
Incorporates patient and support system into treatment and discharge plan.
Provides information to health care team, payors and surveyors while ensuring confidentiality is maintained.
Gives direction to rehabilitation assistant for workflow.
Assume responsibility for professional growth by participating in continuing education, conferences and in-services to provide progressive therapy.
Seeks ways to meet patient therapy needs in a cost-effective quality manner.
5. Maintains equipment needs and condition of equipment to insure proper department functionality.
6. Follows HIPAA regulations and ensures that the confidentiality of patients medical, personal, financial records, employee records, and organizational records is maintained.
7. Participates in FCH committees, quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations.
8. Complies with the state and federal regulatory requirements related to the performance of FCH operations and requirements of the FCH Compliance Program. Including but not limited to:
The Compliance Policy.
Completes all SWANK on-line training annually and as assigned.
All department and organization-wide policies.
Considers the impact of regulations such as Critical Access Hospital requirements, along with Federal and State of Nebraska regulations and discusses concerns appropriately to ensure compliance.
9. Presents self in a professional manner and enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops.
Seeks opportunities for continued growth and performance improvement.
