SUMMARY

Family Service League is seeking a full-time Case Manager for the Regional Engagement and Addiction Program (REAP). The Case Manager will conduct follow-up calls within 30 days of crisis intervention and crisis assessments in a 24/7 crisis stabilization center with a 24/7 hotline and a mobile crisis team.

 RESPONSIBILITIES

  • The Case Manager will conduct follow-up calls on adults, adolescents, and children with the full range of DSM diagnoses (including substance use and mental health disorders) in an acute care, outpatient setting.
  • Provide telephonic and in person crisis intervention, on site and in the field. 
  • Completion of all clinical documentation in accordance with NYS OASAS in an electronic medical record.
  • Provide linkage and follow up to clients during stabilization period and immediately following.
  • The Case Manager will coordinate with outside providers and collateral contacts.
  • Collect data and stats for services and outcomes.
  • General office tasks, including printing, scanning, faxing, emailing, and data entry.
  • Attendance at routine supervision/team meeting.
  • Coordinate treatment with multidisciplinary team.
  • Complete training and maintain proficiency with de-escalation and physical restraint skills.
  • Establish and maintain positive working relationships with others both internally and externally to achieve the goals of the organization.
  • The Case Manager will communicate effectively, both verbally and in writing.
  • Foster Teamwork, work cooperatively and effectively with others to set goals, resolve problems, and make decisions.
  • Assess situations to determine the importance, urgency, and risks, and make clear decisions which are timely and in the best interests of the organization.
  • Set priorities, develop a work schedule, monitor progress towards goals, and track details, data, information, and activities.
  • Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.
  • All other duties as assigned. 

QUALIFICATIONS

Bachelor’s degree required in social work or related field required. Master’s degree in a related behavioral health field preferred. 

Minimum of 1 year of experience in a behavioral health setting (with SUD and MH disorders) required.

Excellent interpersonal, verbal, and written communication skills required. 

Proficient computer skills, including Microsoft Office required. EHR experience preferred. 

Some evening and weekend hours required. 

Valid and clean NYS Driver’s License required. 

Bilingual in Spanish preferred.
