DayOne PACT is Hiring a Service Coordinator - Child & Family Connection Near Lisle, IL

DayOne PACT is a unique not-for-profit agency that provides service case management, trust and guardianship services for people with disabilities in Illinois. This service is through Life Management Services™ program. In addition, we function as the Child and Family Connections agency for DuPage, Kane and Kendall Counties. This service works with families with children who are eligible for services through the Illinois early Intervention system.

We are seeking a full-time Service Coordinator to join our Child and Family Connections team working in our Lisle, IL location. This position generally works Monday-Friday between 8 am – 4:30 pm and requires candidates to be in the office 3 days per week. The ideal candidate will facilitate necessary services to meet the needs of the assigned children. The Service Coordinator is the key contact for the family who has responsibilities to work on behalf of the family and child through eligibility determination, Individualized Family Service Plan (IFSP) development, and service access, provision, and monitoring. The service coordinator will follow ethical guidelines and possess excellent interpersonal skills and good judgement.

Duties/Responsibilities:

  • Provide service coordination and advocacy for a caseload of families with children birth to three years old.
  • Conduct visits with families by traveling to their home or virtually with live video visits.
  • Assess children and families to determine if additional resources or support is necessary using the family directed assessment (FDA) and observation.
  • Develop individualized or family plans to meet identified needs and necessary supports for the child and family. Guide families through the process.
  • Provide support to families and caregivers by implementing meaningful interventions and home programs to be used on a daily basis.
  • Monitor services and outcomes on the IFSP (Individual Family Service Plan) and complete required paperwork, adjust as needed.
  • Effectively collect data in the areas of health histories, social/emotional assessment, growth and development, mental/emotional status, family dynamics, economic, environmental, legal, and cultural.
  • Serve as an integral team member to assure that needed services are provided to families including parent support, community events, playgroups, and developmental evaluations and therapy.
  • Coordinate the delivery of services such as, but not limited to, physical therapy, occupational therapy, speech therapy, and developmental therapy.
  • Conduct follow up on referrals and assist families in making necessary appointments and maintain telephone or written contact with families.
  • Prepare required reports and records such as case notes, documentation of visits, case management contacts, tracking of missed appointments, data base of case load, as well as correspondence to families, physicians, and other service providers.
  • Attend trainings, participate in program and agency meetings, maintain current credentialing requirements.

Required Skills/Abilities:

  • Strong verbal and written communication skills with the ability to interact with individuals at all levels internally and externally.
  • Excellent interpersonal skills, including empathetic, kind, compassionate. Able to effectively listen to clients to identify needs and/or roadblocks.
  • Analytical, organized, efficient, high attention to details.
  • Self-motivated and self-starter that can work with minimal guidance.
  • Ability to function well in a high-paced and, at times, stressful environment.
  • Driver’s license and local travel required. Must meet minimum auto insurance requirements.

Education and Experience:

  • Bachelor’s degree (required) in human service or related field of study, including but not limited to psychology, sociology, education, child development, social work, etc.
  • Prior experience working with children with/without disabilities and/or case management preferred.
  • Experience working with children in a daycare is a plus.
  • Knowledge of available community services a plus.
  • Proficient with Microsoft Office Suite.

Salary: starting at $42,500
