Faithful at Home Care, a premier non-medical home care company servicing several Central Pennsylvania counties from its headquarters in Lancaster, has a Care Coordinator position available supporting the delivery of client services. The company provides personal care and other non-medical services to the elderly and disabled individuals in the comfort of their homes.
This position requires a dynamic, resourceful professional with excellent customer service skills and multi-tasking abilities. Candidate must also have excellent phone communication skills and be a team player. Working in a fast-paced environment this individual will act as the liaison and scheduler between the client and the direct care worker and manage a full caseload. Professional work experience is required, and previous home care experience is extremely desirable.

Responsibilities and Duties:
· Selecting and assigning caregivers to cases and coordinating the delivery of care
· Ongoing communication with caregivers, clients, and other professionals involved in the case
· Supervision and oversight of assigned caregivers providing the services
· Maintenance of caseload, including ongoing scheduling, confirmation of shifts, tracking and managing all service issues or concerns that may arise
· Maintaining automated scheduling system with accurate and current information, including documentation regarding client services
· Acting as the liaison between clients, caregivers, and other professional regarding service issues, including changes in schedules and related matters
· Performing other administrative functions related to the coordination of home care services.
· Reporting daily and weekly performance metrics
· Participate in infrequent on-call rotation for some weekends, evenings, and holiday coverage.
· Support either office location as necessary
Qualifications:
· Bachelor’s Degree required
· Microsoft Office proficiency
· A valid Driver’s License and the ability to travel out of the office as needed
· The desire and drive to grow and learn in this position
· Experience working in a similar role a plus!
· Bilingual - Spanish-speaking
