Description

The following statements are intended to describe the general nature and level of work being performed and are not intended to be a complete list of all responsibilities, duties, and skills required.

SUMMARY:

The Pharmacist is responsible for providing pharmaceutical services within the hospital that has a patient population ranging in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions, with rehabilitation and/or wound care needs. Responsible for implementation of safe medication management processes including selection and procurement, storage, ordering and transcribing, preparing and dispensing, administering and monitoring. The Pharmacist ensures that the pharmacy complies with all state certifications and regulations, Medicare and federal regulations and maintains all accreditation, as well as compliance with all applicable policies, procedures, and standards of the hospital.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Job Specific:

  • Evaluate patients’ home medications.
  • Participates in both the admission and discharge process.
  • Be the "go to person" for the patient and family during their stay
  • Maintains communication with patients, families, and physicians.
  • Must be able to articulate effectively to family and patients and team members.
  • Must use a team approach.
  • Maintains contact with other department heads, medical staff, and nursing staff to determine needs, resolve problems, improve processes, and promote effective drug therapy

Operations and Support Services:

  • Demonstrates satisfactory level of interpersonal skills to interact with facility staff, Administration, patients, families, customers, vendors, and government agencies.
  • Demonstrates and understands the importance of and respect for the rights, dignity, and individuality of each patient in all interactions.
  • Always demonstrates honesty and integrity in the care and use of patient and hospital property.
  • Knowledge of emergency and disaster procedures of the facility.
  • Demonstrates respect for co-workers and responds to needs of patients by complying with hospital policies.
  • Demonstrates ability to prioritize tasks/responsibilities and complete duties within allotted time.
  • Seeks out external resources through conferences, workshops, etc. as necessary. Routinely shares professional knowledge with staff, managers and hospital personnel.
  • Ability to carry out the essential functions of this job (with or without reasonable accommodation).
  • Assure participation in relevant in-service training sessions.
  • Attend and participate in facility meetings as required.
  • Access continuing education opportunities appropriate to responsibilities.
  • Participate in hospital wide patient safety program identifying risks to patient safety and reducing healthcare errors.
  • Perform other duties as assigned.

Qualifications


QUALIFICATIONS:

Education

Requires a bachelor's degree of Pharmacy at minimum. 

License/Certification

At least one (1) year experience in acute care, skilled nursing, long term acute care, or rehabilitation hospital setting required. Current Pharmacist license in good standing for the state of practice. Current Basic Life Support (BLS) through the American Heart Association. Both license and certification must maintain current throughout employment.

Experience

Ability to project a professional image. Knowledge of regulatory standards and compliance requirements. Working knowledge and ability to apply professional standards of practice in job situations. Strong organizational and analytical skills. Working knowledge of personal computer and software applications used in job functions (Word processing, graphics, databases, spreadsheets, etc.).

SUPERVISORY RESPONSIBILITIES:

Pharmacy Technician

OTHER REQUIREMENTS:

Participates in or conducts company-wide sponsored training to safeguard against improper use and disclosure of patient’s protected health information. Monitors and ensures compliance with company policies, procedures, state and federal regulations and Accreditation Standards.
