Erie Family Health Centers is looking for an organized, strategic, and focused leader like you to join our Population Health Team! As the Population Health Manager, you will improve the quality of health through overseeing a large team dedicated to targeted, data-driven outreach to patients either onsite or remotely, as well as ensuring that the patients they serve have the necessary resources to overcome barriers to staying healthy.
Responsibilities
- Create, implement, oversee, and evaluate programs to engage patients in care and assist them in overcoming barriers to staying healthy
- Work directly with the teams leadership to ensure that Erie priorities are aligned with outreach efforts and patient outcomes and experiences with care are improving as a result of the teams work
- Develop outreach protocols for care management, disease prevention, patient re-engagement, and system navigation
- Support staff in managing patient care, including training, follow up, and audits
- Identify potential areas to expand roles and population health initiatives
Requirements
- Bachelors degree in community health or related field; Master of Public Health or Master of Social Work preferred
- 3 years supervisory experience preferred
- Demonstrated cultural competency to work with a low-income, minority population preferred
- Ability to establish and maintain effective working relationships with co-workers, clients, and community partners
- Ability to plan, organize, implement, evaluate, and modify program needs
- Ability to communicate effectively in both English and Spanish preferred (May be required to assist or communicate with non-English/Spanish only speaking patients and/or guests)
