When a child can’t remain safely in their home, we partner with local agencies to support them through foster care, adoption, mentoring and beyond to ensure that children in bad situations move toward brighter futures.Telecommuting/Remote work options may be available Starting salary: $62,000/Year (negotiable based on experience)------------------------------------------------------------------------------------------------POSITION OBJECTIVE: Provide Care Coordination for physical and behavioral health needs for the families who are in in Post Permanency (Permanency Guardianship) Community settings or at risk for entering the dependency system. Provide support for clinical activities within the Utilization Management unit by providing care coordination required by CBC Integrated Health and the Sunshine Child Welfare Specialty plan, clinical consultation, conduct annual caregiver contacts, submit status reports to the court regarding children in the Guardianship Assistance Program (GAP) and Permanent Guardianship, and conduct and track children who have had a Baker Act and ensure appropriate follow up is completed. Monitor children who are verified human trafficking victims to ensure their services and placements are appropriate.KEY FUNCTIONS:
  • Conduct annual follow ups with caregivers of children whose case were closed GAP/Permanent Guardianship or finalized for Adoption.
  • Provide consultation and care coordination to adoptive parents, GAP/PG caregivers, and other professional or paraprofessional staff involved with child(ren).
  • Submit an annual status report to the court that retained jurisdiction for GAP/Permanent Guardianship cases.
  • Monitor and coordinate supports and services for Community Diversion, GAP/Permanent Guardianship, and Post Adoptive children who have had an admission to a children’s crisis stabilization unit through participating and leading, if needed, Local Review Teams, Level of Care MDT, and Baker Act MDT.
  • Provide care coordination to those members on the Sunshine plan and collaborate with CBCIH and Sunshine as needed. Complete documentation in Integrate.
  • Conduct Human Trafficking screenings on children who display risk factors for Human Trafficking.
  • Monitor and coordinate supports and services for Community Diversion, GAP/Permanent Guardianship, and Post Adoptive children who have confirmed or suspected involvement in human tracking through participating and leading, if needed, Local Review Teams, Level of Care MDT, and Human Trafficking MDT.
  • Complete Safe House Assessments on all youth who have confirmed involvement in human trafficking at least every 180 days to assist with monitoring placement.
  • Identify gaps in provider network, coordinates county service needs with provider accessibility, and then communicates findings to Director of Utilization Management and Contracts, to expand network and build provider relationships.
  • Participate in Utilization Management Staff meetings, Operations staff meetings and CMA staff and unit meetings and trainings that are appropriate to care coordination, mental health, Adoptions, GAP/Permanent Guardianship and Human Trafficking.
  • Provide clinical consultation in areas of the Coordinators’ expertise on cases whenever it is deemed appropriate to do so by either the UM staff, Operations staff, CMA staff, the County Executive Director or the Executive Director of Clinical Services.
QUALIFICATIONS: Required:
  • Master’s degree in social work or mental health counseling
  • Minimum of 2 years of experience in working with children with behavioral disorders and placement in out of home care settings.
  • Experience in child welfare
PREFERRED:
  • Licensure preferred as LMHC, LCSW or LMFT
  • Knowledge of the funding & provider resources available in the county and surrounding area
  • Familiar with resources/services supports available for children with substance abuse, mental health, developmental disabilities, and delinquency issues.
SKILLS:REQUIRED:
  • Intermediate data entry skills
  • Proficiency in Microsoft Office Suite including Word, Outlook and Excel
  • Excellent interpersonal and communication skills
  • Strong time-management and organization skills
  • Ability to analyze, integrate and report data
  • Demonstrate cultural & linguistic competence & sensitivity to population served
  • Ability to travel
SPECIAL CONDITIONS: 
  • Thorough Background Screening upon hire including FBI, FDLE, Local Law check, Employment Reference check, DMV check, Criminal Records check, Social Security Trace & Drug Screening. FDLE check will be run at least every 5 years.
  • On-call assignments- Yes
  • Travel- local: 15% per week, long-distance: 5% per month
