Dragonfly Transitions seeks a part-time year-round RN to work closely with our team to coordinate medical needs and medication management for our young adult students.

Dragonfly Transitions supports young adults on a journey of personal exploration, health, and independence. Growth and change happen through connection, community, and experience. Dragonfly is designed with progressive phases, college and vocational options, recreation, therapeutic support, and a variety of living environments located in southern Oregon. Students live in a family style residence where community and interpersonal relationships are encouraged.

The position is in Klamath Falls, Oregon, where outdoor recreation and beauty abound. Our expanded classroom consists of opportunities to enjoy biking, hiking, horseback riding, rock climbing, skiing, snowboarding, and other activities.

Responsibilities

  • Dragonfly duties as required for licensing.
  • Maintain the Master Medication list to include student’s name, date of birth, allergies, list of current medications, supplements, and as needed medications.
  • Update the student’s medication list in their BestNotes file under the medication tab. Track all current, discontinued, and titration schedules on medications and supplements. Complete “Nursing Information” in FORMs tab in BestNotes.
  • Ensure that prescriptions are filled accurately and are up to date according to the doctor’s orders. Track where each student will receive prescriptions, local pharmacy, or mail order. Manage supplements and order as needed. Provide pharmacy with up-to-date insurance information.
  • Establish care with primary care physician (PCP). PCP will prescribe birth control, hormones, inhalers, and thyroid medications.
  • Review lab results and monitor for any follow-up instructions. Document in BestNotes calendar under “medical appointment” and include details in the “notes” section. Track student’s vitals and report to PCP as needed.
  • Create a Medication Administration Record (MAR) for all new students. Track MAR for side effects of new or discontinued medication, daily health checks, patterns with PRNs, missed/declined medications and staff follow up.
  • Maintain a weekly count on all schedule 2 and controlled medications.
  • Maintain a list of approved over the counter (OTC) medications.
  • Dispense medications as needed to assist program campuses.
  • Conduct medication trainings with new staff.
  • Meet with new students and their families to receive medications during the admission process and answer any questions they might have about the medication process.
  • Educate students on the medication(s) they are taking to include the appropriate time to administer, contraindications, and side-effects. Provide training to phase two students when they have been approved to hold their medications and monitor compliance.
  • Ensure that all medications are stored safely and maintain documentation on all medications that have been disposed of.
  • Be in communication with the Clinical Director, Clinical Supervisor and Program Director when errors occur in the medication process. Track medication errors, documentation errors, and follow-up reports monthly (JC).
  • Track Self-Administration of Medication forms to be completed within 4 days of admission (JC).
  • Ensure all new students have a signed the Health Clearance and approved standing OTC orders in BestNotes within the first 30 days of admission (JC).
  • Track student weights bi-monthly and document in BestNotes. Communicate significant weight gain or loss with the therapist.
  • Track COVID and Influenza Vaccine status for all students and staff.
  • Infection Control. Track all students and employees who have been to the doctor for a contagious illness (JC). Collect data around illnesses through the daily health check on the MAR.
  • Coordinate medical appointments for students. Communicate with the doctor around any prescriptions, follow-up care, or recommendations. Obtain approval from parents prior to making appointments to include vision, chiropractic, massage, and dental appointments. Request referrals as needed. Review lab results and monitor for any follow-up instructions. Document in BestNotes calendar under “medical appointment” and include details in the “notes” section.
  • Respond to reports of pain or sickness as they arise and assess the student and determine when a student is to be placed in quarantine due to illness or pain.
  • Monitor the student’s mood, behavior and affect, and document. (Especially when a student starts/stops a medication).
  • Be in communication with parent(s)s to include information around medications, appointments, insurance questions, and be aware of which parent(s) want to be involved in medication changes. Schedule annual dental cleanings.
  • Manage psychiatrist visits. Ensure the initial visit occurs within the first 30 days of admission, second visit 30 days after the initial, and subsequent follow-up visits occur at a minimum of every 3 months. Facilitate appointments weekly or bi-monthly. Document blood pressure, heartrate, and brief summary of the appointment in BestNotes EBH Tx Team section.
  • Communicate any pertinent medical information to the Treatment Team and the Psychiatrist either verbally or in written form.
  • Work a med-pass shift in one house once a month (3-month rotating schedule).
  • Work with clinicians and staff to integrate educational material age appropriate to student population
  • Build new policies/adjust current policies as needed to become compliant with licensing organizations.
  • Rotate with on-call schedule to transport students to Emergency Room when needed and on-call after hours as needed for medication questions.
  • Pack medications for trips, vacations, and breaks; allow extra medications to accommodate delays.
  • Other duties as assigned.

Qualifications

  • The position requires a nursing license (RN) and current CPR certification. Nursing experience is preferred.
  • Minimum of 21 years of age
  • Ability to work as a positive team member through ongoing collaboration with Dragonfly Transitions staff as well as families and outside professionals
  • Superior organization skills
  • A valid driver's license with a clean driving record
  • Must be able to pass a physical exam, pre-employment urine drug test, state and federal background checks
