Elmwood Hills Healthcare Center is currently hiring. Our facility provides residents with a full range of long-term care and rehabilitation services, along with a variety of engaging social and recreational activities. We currently have an opportunity for an energetic and dynamic Social Worker that would enjoy working with elderly, cognitively impaired/ dementia residents.
Qualifications • Bachelor’s degree in Social Work or other human services area such as psychology. CSW or LSW required • At least one year of experience in long term care, with geriatric/elderly population. Excellent verbal and written communication skills • Experience in completion of MDS, CAA and CP, BIMS and PHQ9 • Strong organizational skills as evidenced by the ability to multi-task • Have compassion for the elderly and for their families • Proven ability to work in a team environment • Demonstrated investigation skills related to customer complaints and other resident concerns • Working knowledge of Microsoft Word,
