Full-time opportunity to join our team as a Social Worker. Monday-Friday 9a-5p with alternating late night-Thursday.

We are seeking a dedicated, compassionate, enthusiastic professional who is seeking to make a difference in the lives of children and young adults. The social worker will provide services which support the physical, mental and psychosocial well-being of each resident and their family. 

Responsibilities will include:

  • Case management services for a caseload of 40 plus residents.
  • SW interviews the family and any collateral contacts to complete the initial psychosocial assessment and updates as needed.
  • SW assesses and assists the resident and family with governmental benefits, such as SSI and Medicaid as well as school placement, guardianship and other legal/social needs.
  • Participates as an integral member of the inter-disciplinary and work collaboratively with resident, families, physicians, therapists and nurses to ensure high quality care.
  • Maintain appropriate documentation, by reviewing case notes, logging events and progress in both the EMR and the required Care Plan sections of the MDS in accordance with regulatory guidelines and timelines.
  • Advocates for the mental, physical, emotional, social, educational and spiritual needs of the resident/family.
  • SW works closely with the child welfare system and assists the Interdisciplinary team when concerns of abuse/neglect arise. SW follows all protocols as a state mandated reporter.
  • Prepares resident’s discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
  • In collaboration with the Interdisciplinary Team, SW co-ordinates a safe discharge planning process. Including completion of appropriate referrals for durable medical equipment (DME) supplies, private duty nursing, home health aide, evaluations and assessment for related therapies, pharmacy, WIC and transition to community medical providers.
  • Keeps informed of community resources and makes appropriate referrals to support the psychosocial needs of both the resident and family.
  • Facilitates and encourages resident/family involvement at the Children’s Center. Encourage family participation in care plan meetings and keeps family/legal guardian informed of child’s progress.
  •  Provides palliative care and end of life services to residents and families. Follow up with families for bereavement support and collaborates with Physicians to establish Advance Directives.
  • Attends and participates in the annual mandated training program, and other workshops and training, either in-house or off-site.
  • Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

Other related duties as may become necessary or as directed by supervisor, department director, and/or administrator.

Qualifications

  • MSW from an accredited University in Social Work
  • LMSW or LCSW preferred

Experience

  • Two years of post-graduate experience, including work with families and children, preferably in a pediatric setting.
  • Experience or exposure to medically complex children and their families

Salary Range: 

$62,000/Yr to $68,000/Yr
