Edward M. Kennedy Community Health Center is Hiring a Clinical Care Manager-RN Near Framingham, MA

What’s a Clinical Care Manger?

A clinical care manager is a healthcare professional who assesses patients' needs and creates personalized care plans. Clinical care managers oversee day-to-day operations in a clinical setting, evaluate and manage medical assistants, write policies and procedures, and help educate and train personnel. Their clinical duties include taking and recording vital signs, medical histories, preparing patients for examination, drawing blood, and delivering medications as requested by a physician. They are also passionate about equitable health outcomes.

Our Clinical Care Managers:

  • Have bachelor degrees in nursing from an accredited college or university
  • Are registered nurses licensed in Massachusetts
  • Have at least 1-2 years of clinical experience
  • Hold a current CPR certification
  • Are willing to be fully vaccinated for COVID19 and Flu

What will I do?

As a clinical care manager with a registered nursing license in the state of Massachusetts and 1-2 years of clinical experience, you will:

  • Work with Primary Care Providers (PCP) coordinating and managing the care of vulnerable patients, making sure patients receive the best care. This means working with severe illness, chronic disease management, education about lifestyle, behavior changes, and preventive care in different health settings with many providers.
  • Manage care plans for patients admitted to and released from the hospital, patients seen in the emergency room, and patients coming from or going to another health care facility with the goal of preventing disease complications, improving results, increasing patient participation in self-care, decreasing risk, and reducing hospital and ER use.
  • Identify, manage, and coordinate patient care and provide ongoing updates to the care team about patient care, needs, and plans of care.
  • Provide patient care which includes, but is not limited to, patient assessment, delivering of vaccinations and medications, medication reconciliation, point-of-care testing, treatments, and lab and diagnostic test result reviews.
  • Document patient care, ongoing evaluations, patient progress/risk status, care manager interventions, and a plan of care developed with the Primary Care Physician (PCP), in the EHR System according to health center and nursing standards.
