Ecumen Home Care-Pathstone is hiring a Private Duty Registered Nurse, RN of Home Care. Ecumen Home Care-Pathstone provides in-home and community based services in the Minnesota Mankato area.

Responsibilities:
Private Duty Registered Nurse (RN)
The Registered Nurse is responsible for providing direct care to a vent dependent client living in his/her home, in accordance with agency standards and policies. Perform skilled nursing functions in the home and community setting as well as serve the nursing needs of the client. Specific responsibilities include:
  • Completes and documents Nursing assessments for Private Duty client living in his own home.
  • Collaborate with physicians, other health care professionals, client and family in developing a comprehensive coordinated plan of care.
  • Deliver skilled services efficiently and effectively to meet patient care needs.
  • Ensure that physician orders are communicated to the agency's staff with accuracy and thoroughness and understanding.
  • Promote personal safety and a safe environment for client and co-workers.
  • Other duties as assigned.
Qualifications:
Registered Nurse (RN)
Minimum Required Qualifications:
  • Education: Graduate of an accredited Registered Nursing program
  • Relevant Experience: One to two years in post-acute care setting
  • Licensure/Certification: Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.
  • Current valid driver’s license and proof of vehicle insurance
  • Ability to communicate effectively in both written and verbal formats
  • Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA< Resident’s Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations.

  • Education:
  • Relevant Experience: One to two years nursing experience in Medicare-Hospice
  • Licensure/Certification:
  • Other knowledge, skills and abilities: (examples include supervisory, computer skills and software/hardware knowledge, etc.).
