The Federal Bureau of Prisons is awarding an indefinite delivery, requirements type blanket purchase agreement with firm fixed unit prices to a responsible entity for the provision of a Dental Hygienist to provide services to the inmate population at the Federal Correctional Institution (FCI)/Federal Prison Camp (FPC) Phoenix, Arizona.
Dental Hygienist services will be provided in the FCI/FPC Phoenix Health Services Department located at 37900 N. 45th Ave, Phoenix, AZ, 85086. The duties of this position require frequent and direct contact with individuals in confinement who are suspected or convicted of offenses against the criminal laws of the United States.
Work hours may vary depending on the need of the institution. Shifts will typically take place between the hours of 6:00 AM to 3:00 PM. The shift will be up to 8 sessions per day, with a thirty (30) minute unpaid lunch period, if applicable, Monday through Friday, exclusive of Federal holidays. A session will equal one (1) hour. The schedule shall not exceed forty (40) sessions per week. The Health Services Administrator (HSA) and/or designee will be responsible for defining the weekly schedule. These schedules are subject to change based on the variable needs of the institution and the unpredictable nature of the correctional environment. Upon agreement, any schedule changes must be coordinated with the HSA.
Due to the nature of the correctional setting, delays in performance of work may be experienced. Such delays may include but are not limited to inclement weather, electrical or mechanical problems, or other security matters. When contract employees report to the institution but are unable to perform services due to such delays, the contractor will be paid for (2) sessions.
Normally, any remaining sessions for that day will be suspended and the contractor will be free to leave and the contractor will not be paid for remaining unperformed sessions. However, if the institution requests that the contractor remain at the institution for eventual performance, the contractor shall be paid for such sessions.
The contractor shall notify the Health Services Administrator and/or designee regarding arrangements for coverage in their absence not less than twenty-four (24) hours in advance, by telephone.
The institution will provide all supplies and equipment the contractor may routinely need. All records maintained by the Contractor(s) under this contract will remain the property of the institution. The Contractor(s) is expected to wear professional clothing (scrub uniforms are approved, no denim, no orange, no khaki).
The contractor shall provide the following Dental Hygienist services, which include, but are not limited to, the items listed below:
The contract employee shall immediately inform, in writing to the HSA and Administrative Contracting Officer, any adverse action taken by any regulatory body involving certification. This includes, but is not limited to, suspensions, revocations, restrictions on licensure as a Dental Hygienist.
The contractor will be responsible for personal health care needs and costs associated with exposure, or possible exposure to any blood borne pathogen or other risk encountered during performance of this contract.
The services provided shall be professional in nature, and will be commensurate with AAACH standards of care. The methods utilized will meet medical practices established in the local community and adhere to all applicable federal, state, and local regulations. These services, although not directly supervised, will be reviewed periodically be the Health Services Administrator (HSA) and the Chief Dentist at FCI Phoenix. The HSA will retain professional responsibility for all patients referred to the Health Services Department.
The contractor has sole responsibility and discretion for choosing and implementing the manner and method of performing tasks outlined in the statement of work as long as the requirements are met in accordance with the regulations described above.
The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of services rendered. (e.g., professional judgments, diagnosis for specific medical treatment). Contract monitoring reports will be prepared by the Contracting Officer’s Representative (COR) and maintained in the contract file.
The Contractor shall ensure that the Dental Hygienist is free of infectious diseases. Any individual performing as a Dental Hygienist under this contract who subsequently develops a medical condition, which might reasonably be expected to place other workers, patients, or the public at risk shall be immediately removed from activities requiring performance of invasive procedures and activities, which might reasonably be expected to place other workers, patients or the public at risk. In any such case, the Contractor shall be responsible, at no additional cost to the institution, for securing the services of any replacement personnel required to perform this contract.
The Contractor is required to be certified/licensed as a Dental Hygienist in any State and must submit photocopies of their license and/or board certification. Dental radiology certification and anesthesia certification are required. CPR certification is also required. In addition, a minimum of three years of experience is preferred.
