DUNBAR COMMUNITY COUNSELING SERVICE
CARE COORDINATOR

GENERAL OVERVIEW OF KEY ROLES AND RESPONSIBILITIES:
  1. The primary responsibility of the IBHS Care Coordinator provides supports to families as an adjust to the clinical treatment. Care Coordination includes assessing children’s overall wellbeing and providing supports to children and families to address identified physical and behavioral health needs .The Care Coordinator should primarily be responsible for engaging children, their families, and other significant persons in a collaborative relationship to promote positive outcomes.
  2. Ongoing support and resources through a wide range of quality assurance, planning, supervision oversight and implementation of Dunbar’s policies and procedures. The overall goal of the position is to enhance the quality of consumer management and satisfaction; as well as to promote the stability of care through case coordination and administrative operation, as well as integration of the System of Transformation—Recovery and Resiliency principals. The Clinical Care Manager holds accountability for the care and coordination of all patients.
  3. This position has both Supervisory and Care Management Components.
Essential and Core Functions:
  1. The Care Coordinator should spend a considerable amount of time assessing and evaluating the social determinants of health related to each family.
  2. The Care Coordinator should be well-versed not only in supports and resources available throughout the City of Philadelphia but also within the local community surrounding the child’s school and home.
  3. They should identify individual family needs and interface as needed with other relevant systems to connect families to resources in the community when appropriate. They should also work as part of the service team in the implementation of service plans with goals of retaining or re-engaging the child when needed.

v. Responsible to work with Clinical Supervisor and Administrative Director to ensure that all staff complete initial and annual training requirements.

  1. Introduces self to consumer and family and explains Care Coordinator role and process for patient and family.
  2. Keeps thorough records of each of case, including documentation of all calls, and resource referrals.
  3. Responsible for ensuring the delivery of authorized and prescribed services.
  4. Act as consumer advocate and resolve general complaints.
Essential and Core Functions Cont’d:
  1. Reaches out to consumers to assess their most urgent needs (ongoing).
  2. Conducts review for appropriate operation of services from admission through discharge.
  3. Evaluate consumer satisfaction and quality of care provided.
  4. Maintains the confidentiality of consumers.
  5. Maintain ongoing contact with consumers.
  6. Quality review updates.
  7. Audit charts.
  8. Staffing cases in a timely manner
  9. Send out appropriate documents to new staff.
  10. Help manage Psychiatrist’s schedule and collect appropriate paperwork.
  11. Manage Camp spreadsheets.
  12. Manage electronic Case Load and Productivity files.
  13. Daily review and follow-up productivity.
  14. Responsible to facilitate contracted addendums and Doctor’s review.
  15. Responsible for all denials and notification forms.
  16. Review all incoming paperwork.
  17. Manage A&A and School Provisional spreadsheets.
  18. CBH referrals response and feedback.
  19. Participate in Monthly provider’s meetings.
Organizational Accountability and Relationship:
  1. To be supervised by the Administrative Director, Clinical Director and CEO
  2. Keep records and assist Clinical Supervisor with the management of individual and group supervision
  3. Engage in training support, as needed.
  4. Connect families to resources.
  5. Review and adhere to IBHS regs and Agency policies and procedures
  6. Assist the BHRS Director in staffing cases.
  7. Review Paperwork all paperwork for quality assurance and policy compliance
  8. Conduct weekly compliance audits for IBHS charts
  9. Assist the IBHS Director in quality clinical oversight of the IBHS department

Education and Experience:
  1. A Care Coordinator shall meet all of the following:
    • Bachelor degree in Psychology, Social work, or other related field. 
    • Minimum of one (1) year of related experience.
    • Possess good oral and written communication skills with people of all demographics, interpretation capabilities, counseling techniques, problem solving skills, and appropriate decision making skills.
    • Possess conflict resolutions skills and needs to be able to perform well under difficult situations. 
    • Ability to operate a personal computer, fax machine, printer, and copier.
    • Access to reliable transportation
  1. Valid child Abuse, criminal history, CPR/ First Aid and FBI clearances
