Dragonfly Transitions seeks a full-time RN or LPN to work closely with our team to coordinate medical needs and medication management for our young adult students. This position is ideal for nurses who are passionate about making a difference in the young adult population who struggle with mental illness. The Homestead offers a “softer” landing for students in the setting of a family style residence where community and interpersonal relationships are encouraged. The Homestead can house up to 6 students and is co-ed. This campus has a large greenhouse, barn, chicken coop, and garden on the 30-acre property.

The position is in Klamath Falls, Oregon, where outdoor recreation and beauty abound. Our expanded classroom consists of opportunities to enjoy biking, hiking, horseback riding, rock climbing, skiing, snowboarding, and other activities.

Responsibilities

· Take vital signs and asses for medical and mental health needs daily.

· Provide non-emergency medical care including basic minior wound care.

· Establish and maintain client medical chart.

· Manage admitting and weekly appointments with Psychiatrist.

· Establish care and coordinate with local primary care physician (PCP) for medical and prescription needs including birth control, hormones, inhalers, and thyroid medications.

· Be able to make recommendations based on student symptoms to PCP and review diagnostic test results and initiate orders from provider.

· Follow infection control protocol and be able to investigate and track protentional outbreak of illness.

· Maintain the Master Medication list to include student’s name, date of birth, allergies, list of current medications, supplements, and prn medications.

· Create and manage MAR for all students.

· Be able to identify potential adverse effects of new medication prescribed by PCP and/or Psychiatrist.

· Ensure that prescriptions are filled accurately and are up to date according to the doctor’s orders. Track where each student will receive prescriptions, local pharmacy, or mail order. Manage supplements and order as needed. Provide pharmacy with up-to-date insurance information.

· Maintain a weekly count on all schedule 2 and controlled medications.

· Maintain a list of approved over the counter (OTC) medications.

· Dispense medications as needed to assist program campuses.

· Conduct medication trainings with other staff.

· Ensure that all medications are stored safely and maintain documentation on all medications that have been disposed of.

· Be in communication with the Clinical Director, Clinical Supervisor and Program Director when errors occur in the medication process. Track medication errors, documentation errors, and follow-up reports monthly (JC).

· Be in communication with parent(s)s to include information around medications, appointments, insurance questions, and be aware of which parent(s) want to be involved in medication changes.

· Work with clinicians and staff to integrate educational material age appropriate to student population.

· Build new policies/adjust current policies as needed to become compliant with licensing organizations.

· Support transportion of students to Emergency Room if needed and on-call after hours as needed for medication questions.

· Pack medications for trips, vacations, and breaks; allow extra medications to accommodate delays.

· Support the therapeutic milieu with direct student support and group facilitation as needed.

· Other duties as assigned.

Qualifications

· Oregon RN license or LPN license

· CPR certification.

· Nursing experience is preferred.

· Minimum of 21 years of age

· Ability to work as a positive team member through ongoing collaboration with Dragonfly Transitions staff as well as families and outside professionals.

· Superior organization skills

· A valid driver's license with a clean driving record

· Must be able to pass a pre-employment urine drug test, state, and federal background checks.
