Distinctive Healthcare is currently seeking to hire full time LCSW to work at the military hospital at the Seymour Johnson AFB in Goldsboro, NC.

Responsibilities

The health care worker (HCW) shall serve as a member of the primary careclinic’s health care team and assist the primary care manager (PCM) in managingthe overall health of their enrolled population. The program goal is to assist theAir Force Medical Home (AFMH) in meeting its core principles of “whole personorientation” and “coordinated/integrated patient care” by improving managementof cognitive and behavioral factors impacting the health of the enrolledpopulation. Scope and responsibilities include patient care (e.g., functionalassessment, targeted and collaborative problem solving) and consultation withAFMH members to identify (and efficiently and effectively target) primaryproblems impacting the enrolled population. Services will be governed by US AirForce practice standards as delineated in the Primary Care Behavioral Healthpractice manual.

Qualifications

5.2.1. Must hold a Master of Social Work (M.S.W.) degree from a graduate school ofsocial work fully accredited by the Council on Social Work Education (CSWE).5.2.2. Maintain a current, unrestricted clinical license to independently practice socialwork in any one of the 50 states, the District of Columbia, Puerto Rico, or theU.S. Virgin Islands. In addition, must maintain credentials and privileges withoutlimitations or restrictions.5.2.3. Must have at least 12 months of experience, in the last 24 months, with assessingand providing evidence-based individual counseling to patients within a mentalhealth or health care facility.5.2.4. Must have at least 12 months of direct hands-on experience in the last 24 monthswith computer operations, basic word processing, data entry, and use of anautomated medical record system.5.2.5. Prefer those with recent experience (within the past three years) providingintegrated behavioral health services within a primary care setting.5.2.6. Must be clinically and culturally competent to work with the military population,both active duty and beneficiaries (child, adolescent and adults).
