Summary
SUMMARY: Under the direction of the Department Director, the role of the ED case manager includes all the functions of core Case Management including assessment, planning, implementation, coordination, monitoring and evaluation of options to meet the patient's health needs in the emergency room, transition to admission and post ED discharge needs. The ED Case Manager assesses patients for present-on admission (POA) conditions to ensure accurate documentation regarding hospital acquired conditions.
EXPERIENCE: Previous experience as a case manager with emergency department experience preferred.
LICENSE/CERT: Required: RN (Registered Nurse) State of Missouri Preferred: ACM (American Case Management) within 1 year of hire
EDUCATION: Required: Bachelors - Nursing
