Job Summary

At Desire for Healing, Inc., quality resident care is delivered by a team of personnel, each with their own specialties. The Resident Care Coordinator (RCC) plays a daily vital role in this to ensure continued good overall health of each and every resident.

Job Description

Responsibilities of the RCC position include:

· Ensuring the continued good health of our residents

o Taking appropriate action when changes in resident’s health

o Coordinates resident care with outside agencies and medical providers

o Coordinates resident appointments with support from office staff, and occasionally attends appointments with residents if needed

o Reviews meal logs, vitals and weights and communicates with RN, Assistant Administrator and Executive Director of any changes or variances, and/or any issues with resident nutrition

o Responds to resident emergencies, provides first aid assistance, and arranges for appropriate medical attention and follow-up

o Communicates with medical providers, home health, and hospice teams

· Ensures State Compliance

o Ensures resident Service Plans are up to date, read and being followed by care staff

o Participates in the creation of, updating, and reviewing of Service Plans

o Reviews and responds to nursing and pharmacy audits and support

o Reviews and documents regularly in the 24-Hour log and at least weekly on each resident in the electronic health record

o Enters, reports on, and follows-up on Incident Reports

o Creates, monitors, and charts Temporary Service Plans

o Charts on resident medical appointments, hospital stays and discharges, medication orders

· Med Room

o Manages Med Tech staff

o Responsible for Med Tech training and ongoing training to comply with regulations and policies

o Recruits, interviews, hires, delivers performance reviews, and shift coverage for Med Techs

o Oversees all activities pertaining to the Med Room and medications (orders, MARs, TARs, narcotics log, resident Charts, cycle-fill, etc.)

o Supervises Med Room supplies and equipment ordering

o Coordinates diabetic supplies needed for residents including the ordering and organization of supplies

This position is responsible for adhering to Oregon State regulations with regards to Residential Care Facilities and Nursing. All the Oregon State requirements will be followed and kept in order, easily accessible, and easy to find/read.

RCC Requirements

· Certificates in CPR, and First Aid

· Familiarization with nurse regulations for Residential Care Facilities

· Pleasant demeanor to communicate with residents, staff, medical personnel, families, etc.

· Good communication skills

· Professional appearance

· Experience with PointClickCare a plus

· Excellent organizational skills

· Medical and/or Long-Term Care experience a plus

  • Weekend availability

Ability to commute/relocate:

  • Pendleton, OR 97801: Reliably commute or planning to relocate before starting work (Required)

Experience:

  • Medical: 3 years (Preferred)

License/Certification:

  • CPR Certification (Preferred)

Shift availability:

  • Day Shift (Preferred)
