Position Details
The Intensive Transition Service (ITS)Licensed VocationalNurse(LVN)is a key team member responsible for various rehabilitation and support tasks, assisting in behavioral health treatment, substance abuse services, education, family consultation, and crisis intervention. Working under the clinical supervision of the Program Director and Psychiatrist, the nurse's primary focus includes medical monitoring and education for clients served by the ITS team. Collaborating closely with Psychiatric Care Providers, Program Directors, team members, and Extended Treatment Teams is essential.
Job Description

• Coordinate and monitor all medical and psychiatric medications, including metabolic screenings and vital signs.
• Maintain thorough documentation of patient outcomes and medical conditions, coordinating care with various healthcare providers.
• Provide medication education and leadership in medical and mental health treatment.
• Conduct regular health assessments, document patient progress, and deviations from care plans accurately.
• Engage in nursing administrative duties, support goal development, and aid in creating specialized health programs.
• Deliver recovery-focused services, develop comprehensive goal plans with individuals and their families, and coordinate care within treatment teams.
• Act as a liaison with community agencies, support families, and ensure rights and preferences are respected.
• Conduct thorough assessments, deliver treatment and rehabilitation services, and actively engage in organizational meetings and cross-training sessions as needed.

Earn $28 - $32/ hour.

Requirements
The ideal candidate will possess the following qualifications:

• Must hold a current unrestricted RN or LVN License in CA
• Experience in either the intellectual developmental disabilities or behavioral health preferred
• Must possess a Valid CA Drivers License - Periodic Travel to various sites will be required.
