Skybird Landing Apartments, a Single-Site Supportive Housing Program, is looking for a passionate mental health professional to join our team! The Housing Specialist is responsible for the oversight of clients enrolled in the Supported Housing portion of the SP SRO housing.

DIRECT SERVICE RESPONSIBILITIES:

1. Develop, monitor and update housing support plans to identify which services DePaul is to provide to maximize clients success.
2. Assists in linkage to any necessary support services such as mental health treatment, medical, vocational and educational services.
3. Networks with support services such as mental health treatment, medical, vocational and educational services.
4. Assists clients in accessing and keeping all financial entitlements they are eligible for.
5. Maintain accurate, up-to-date records.
6. Assists the other staff in the building by performing essential duties in their absence.
7. Performs other duties as assigned.

ADMINISTRATIVE RESPONSIBILITIES:

• Attend county SPOA meetings as part of DePauls intake team to help with admission decisions.
• Assess referrals to determine if SP SRO is an appropriate level of care for individuals.

Education: Bachelors degree or 4 years of mental health and/or specials needs population experience (per site needs).

Experience: One year working in human services field preferred; or equivalent combination of education and experience.

Other: Valid New York State drivers license. Must meet qualifications set by agencys clean driver record policy. Use of personal vehicle required.

Skybird Landing Apartments is a New York State Office of Mental Health funded program that provides long-term or permanent housing where residents can access the support services they require to live successfully in the community. Skybird Landing Apartments has a total of 60 units; 30 units are reserved for the Supportive Housing Program.
