Advanced Practice Nurse ( Family Practice)

Time to invest in some new footwear because if you’re San Antonio-bound, you’d better show up in boots! SeaWorld, the San Antonio Zoo, Six Flags Fiesta Texas, and downtown museums are all fun outings for kids and families. Tube or kayak down a river, play or fish in the lakes, or relax by San Antonio’s famous downtown River Walk. Museums, art galleries, and historical landmarks—remember The Alamo?—round out San Antonio’s cultural offerings. Tex-mex cuisine arguably originated here, and some up and coming culinary trendsetters are making a name for themselves in the Pearl District downtown, alongside several notable breweries.

Responsibilities

The purpose of the work is to ensure a high level of health care for patients with chronic and acute illnesses. The incumbent is responsible for facilitating the medical care of the patients in the outpatient setting, in addition to developing the plan of care. This includes participation in patient education in order to promote the effective involvement of patients in their care. The work engages in Department of Army research with immediate and long-range impact through planning, implementation of care and evaluation, patient teaching, family counseling and involvement with interdisciplinary colleagues in the advancement of medical treatment initiatives.MAJOR DUTIES:Employee provides independent and collaborative specialty, comprehensive and therapeutic health care services, integrating the art and science of nursing into medical practice. Medical management functions performed include assessment of acute, non-acute and chronic clinical and psychological diagnosis, new patient intake/screening, history and physical examination, laboratory and diagnostic studies; to include interpretation, ordering appropriate medications and treatments, providing staff education, and the initiation of a therapeutic plan in accordance with medical practice. Formats a therapeutic plan of care in collaboration with specialty physicians. Implementation includes major emphasis in the areas of patient/family education, assessment, comprehension, and support. The incumbent is responsible for the initiation, implementation and supervision of treatment plans and education for chronic conditions such as; but not limited to, coronary artery disease, endocrine diseases such as diabetes, hyperthyroidism, obesity, hyperlipidemia, and chronic kidney disease. Collaborates with physicians and other team members regarding patient management, to include evaluating and revising therapeutic plans.Provides educational management to patients and their families regarding disease processes and diagnostic tests; the therapeutic regimen including medication, diet and follow-up care; methods of adapting medication and treatment to current lifestyles. Provides counseling to patients and their families for prevention of illness and maintenance of health, in order to assist patients in attaining physical, mental, and social well-being. Oversees compliance with the preventive healthcare needs of adult patients visiting the cinic. Refers high-risk, unstable, or extremely complex patients to the physicians and emergency medicine. Performs emergency care as appropriate.Incumbent will be presenting patient cases and classes for patient care conferences in collaboration with other members of the health care team. Communicates patient management strategies to members of the patient care team. Collaborates with physicians regarding patient management, to include evaluating and revising therapeutic plans. Serves in a leadership position and is recognized as a consultant and role model to other health care professionals both within, and outside, the institution. Develops and/or conducts in-service education programs for other health care professionals. Serves as advisor/member of committees as needed. Develops, reviews, and appropriately updates policies, protocols, standards of nursing practice, standard operating procedures and scope of practice regarding adult, adolescent, pediatric and geriatric patient care in collaboration with the multidisciplinary members of the health care team.

Qualifications

US Citizenship required

At least one year of clinical experience AFTER finishing APRN program

Applicants must have passed the National Council Licensure Examination and possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
