David's Loft Clinical Programs is Hiring a Community Support Specialist Near Baltimore, MD

Scope:

At David’s Loft Clinical Programs, we owe our success to people and processes. A Billing Specialist is a supportive force behind both, and we are currently seeking someone exceptional to take the helm. The ideal candidate will assist individuals with disabilities to gain skills necessary to live, work, and recreate based on the individual’s personal preferences. The Community Support Specialist will work directly with the individual with disabilities to learn coping and daily living skills. Community Support Specialist will work with employers as well as provide on-site interventions to help support individuals to adjust to the workplace and the routine of maintaining employment. Community Support Specialist provides continuous monitoring to the supported individual and job site to assess the needs of the supported individual and job site offering support when needed.

Status: Non-Exempt or Contractual

Responsibilities: 

  1. Network with local businesses/ employers to develop employment opportunities for individuals with disabilities
  2. Provide vocational counseling and job coaching to individuals
  3. Complete job/task analysis; including developing and implementing job training plans
  4. Monitor job placements with a goal 90-180 days retention
  5. Visit job and internship sites and meet with individuals with disabilities on-site, as scheduled; including evenings and weekends
  6. Provide intensive skills training and employment adjustment counseling at the job site
  7. Monitor individuals work adjustment through direct observation and through meetings with employers and supervisors
  8. Meet with individuals for follow-up counseling at the agency site
  9. Serve as a member of the multidisciplinary behavioral health team
  10. Monitor supported individual for signs and symptoms of relapsed
  11. Assist supported individuals with securing housing and other resources and supports necessary for independence and self-sufficiency
  12. Develop, monitor, implement and document employment and rehabilitation treatment planning goals
  13. Develop and prepare written assessment of individuals adjustment to work and file them in case records
  14. Address problems and implement strategies as outlined in the individual treatment plans of program participants
  15. Plan and engage in activities that promote rehabilitation and wellness
  16. Document contacts, problem areas addressed, and implemented strategies with program participants
  17. Ensure compliance with ongoing mental health treatment
  18. Assist in securing community and generic resources for program participants and families
  19. Document interactions, progress etc. in the individual’s case record
  20. Attend case conferences or interdisciplinary team meetings and training; and development workshops
  21. Other duties as assigned as needed to drive our organization's mission, vision, and values

Requirements and Qualifications:

  1. Possess an Associates Degree in human services or equivalent education/experience
  2. 1-year experience in case management, mental health counseling or substance use disorder related counseling
  3. Excellent time management skills and ability to multitask and prioritize work
  4. Attention to detail and problem-solving skills
  5. Excellent written and verbal communication skills
  6. Strong organizational and planning skills
  7. Proficiency in MS Office.
  8. Minimum two (2 ) plus years of experience providing direct support to adults with severe persistent mental illness and substance abuse disorders. 
  9. Peer Recovery Support Specialist REQUIRED

Director of Rehabilitation Services

Physical Capacity:

Must be able to sit and stand for 20 minutes and lift at least 10 lbs.
