Resident Wellness Director – South Boston

CSI Support & Development Services is looking for a organized, flexible, knowledgeable candidate to join our services department in supporting the members of our consumer cooperative as they live, play and volunteer in our superior and affordable senior housing communities. The RWD will engage with members in a comprehensive, proactive and on-going manner to assess and identify member needs, develop and implement individual healthy aging plans, actively motivate and educate members on self-management of chronic conditions, and promptly implement transition plans in close collaboration with hospitals and nursing homes. Employing a person-centered approach, the RWD works in concert with each participant and their natural support network. The RWD’s responsibility is to help members address needs, achieve their self-identified goals, and support their ability to safely age in place.

Job Duties and Responsibilities include:

• Serve as the center coordinator and communicator for the Supportive Services Demonstration (SSD) within the co-op.
• Enroll members in the supportive services demonstration, including explaining all privacy and confidentiality protections.
• Assist and educate members and their families who have requested support in enhancing the quality of their lives.
• Oversee an initial and on-going resident assessment process, including conducting person-centered interviews to understand member lifestyles and interests, and collaborating with the Wellness Nurse (WN) to complete a member health and wellness assessment that gathers information about member physical, mental, functional and social status.
• Act as a liaison between community agencies, service providers and members.
• Collaborate with the SSD Implementation and Evaluation Teams, including participating in all trainings, to the greatest extent possible; engage in peer-to-peer learning activities; engage in a quality assurance process; and participate in interviews, surveys, or other evaluation activities.
• Establish relationships with local service providers who are currently providing or could provide services to members and monitor the quality and effectiveness of the service deliver.
• Plan and implement dynamic educational and group programs.

Job Qualifications:

• Bachelor’s degree or higher in Social Work, Psychology, Gerontology, Counseling or related specialty with 1 years of experience working with older adults in a community-based setting;
• Strong oral, written and interpersonal skills.
• Cultural competency in working with diverse populations and individuals from different ethnic and racial backgrounds.
• Experience using motivational interviewing, active listening, and person-centered care concepts to help encourage individuals to engage in addressing and managing their own care needs.
• Proficiency in Microsoft Office Products
