Crystal Home HealthCare is Hiring a Registered Nurse Case Manager Near Detroit, MI
Seeking a Full-time or Part-time registered nurse to make home healthcare visits. The RN will be responsible to create the plan of care, start of care, evaluate and resumption of care (if ordered by the pcp). The RN Case Manager will be responsible for the patients care until discharge from the agencies care. RN Case Manager will be responsible for supervision of the LPN and HHA throughout the plan of care. Must have a Valid RN license in the State of Michigan, have one year of hospital/facility experience, home healthcare is a plus!
