Crossroads Treatment Centers is Hiring a Care Manager Near Meadville, PA

Company Description:

Crossroads was founded on May 5, 2005 to serve a patient population battling with opioid addiction. Since its founding in 2005, we have been a physician led healthcare organization. The company has grown rapidly and now includes 90 treatment centers in 10 states. Each member of the Crossroads team specializes in an area that supports the recovery of over 26,000 patients, monthly. Crossroads' clinical staff includes physicians, pharmacists, counselors, nurses, and other service coordinators. Along with medication management, our staff works closely with each patient to create an individualized treatment plan aimed at building recovery and relapse prevention skills.

***Must be willing to Clarion, Meadville, Franklin, and Greenville***

General Responsibilities:

Promotes and restores patients' health through best practices care coordination by collaborating with medical providers and multidisciplinary team members.

Requisite Skills, Knowledge, and Experience:

  • Candidate must possess a High School Diploma.
  • Lived addictions experience is a plus.
  • Demonstrated interpersonal skills and the ability to work independently and as a member of a team.
  • Demonstrated competency communicating effectively with substance abusing individuals and families.
  • Demonstrated reading and writing skills.

Specific Training Competencies:

  • Identifies patient care requirements by establishing personal rapport with patients and other persons in a position to understand care requirements.
  • Establishes a compassionate environment by providing care to patients, friends, and families.
  • Promotes patient with Opioid Use Disorder (OUD) to health by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills and harm reduction; answering questions.
  • Assures quality of care by adhering to Center of Excellence (COE) standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following agency's philosophies and standards of care and governing agency regulations.
  • Resolves patient with OUD problems and needs by utilizing multidisciplinary team strategies.
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel.
  • Documents patient care services by charting in patient records.
  • Maintains data for quality improvement and quality control efforts; produces reports as needed for COE audits.
  • Conducts triage of acute and chronic clinical needs of patients via telehealth (audio and visual) or telephonic (audio only) or in the centers; able to troubleshoot and utilize protocols for scheduling appointments, medication refills, clinical care and treatment.
  • Monitors clinical status of patients, e.g., medication adherence and referral follow-ups.
  • Maintains continuity among staff by documenting and communicating actions, irregularities, and continuing needs.
  • Maintains patient information confidentiality and privacy.
  • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
  • Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods. This is a strong multi-disciplinary team environment.
  • Contributes to team effort by accomplishing related results as needed.
  • Physically work from one assigned center when provider is present to provide real-time assistance to patients for identified psychosocial needs.
  • Physically work from community locations such as Emergency Departments, Addiction Assessment Centers, Jails, and other places where "warm hand-offs" and bridging activities can occur. Performs this work during times when a provider is not seeing patients in the center. This includes maintaining the relationship between outside agencies and Crossroads.
  • Receive requests for ROI's from Clinicians, In-Patient and Outpatient Rehabs, Therapy, PCP and emergency medical visits.
  • Receive and promptly address daily referrals from physicians in EMR with such topics as Hep C, higher Level of Care (LOC), and housing.
  • Monitor clinical outcomes dashboard, receive emails regarding patients at risk for suicide, reach out to said patient and refer where appropriate.
  • Assist physicians and clinicians in onboarding patients and orienting patients.
  • Coordinate care of patients entering in-patient rehab and helping to facilitate Tele-health visits while patients are admitted and assisting with continuation of care upon discharge of facilities.
  • Assisting patients with therapy available in their area or within Crossroads.
  • Problem solve questions from patients or staff, e.g. problems with pharmacy, insurance, prior authorizations.
  • Assist with specialty projects pertaining to the overall improvement and quality of care.

Mathematical Skills: Ability to calculate figures and amounts such as percentages, mean, mode and median ability to draw and interpret graphs.

Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions.

The employee occasionally lifts/carries paperwork, files, office supplies, up to 20 lbs., pushes/pulls exerting up to 5-10 lbs. force, opening doors, file drawers, pushing files in drawers, and kneeling/crouching and/or ability to reach lower file drawers, file stretching to store of retrieve materials. Specific vision abilities required by this job include close vision and the ability to adjust focus.Frequent sitting at desk or computer while typing, completing paperwork, or meeting with patients.
