Experience & Requirements
* High School graduate or equivalent.
* Minimum one-year medical records experience.
* Familiarity with the Problem Oriented Medical Record (SOAP) system essential.
A background check, security clearance and drug screening are required for this role.

Responsibilities
Below is a list of your responsibilities as a Medical Records Clerk with YesCare at (Facility Name) located in (City, State):
* Retrieve medical charts for all healthcare staff or clinicians as requested. File daily all in use medical records. Secure all active and inactive medical records.
* Assure that charts are counter-signed by Physician, check charts for completeness.
* Release information at the direction of the Medical Records Supervisor, Medical Director or Health Services Administrator.
* Answer the telephone, take messages and make telephone calls. Type letters, reports and memorandums.
* Maintain a roster or appointment book for all scheduled patient appointments both on and off-site.
* Order, receive and maintain office supplies.
* Perform other duties as assigned.
