CoreLife enhances total health by providing a multifaceted approach to obesity, chronic illnesses, their root causes, and related side effects. The Nurse Practitioner is a licensed independent practitioner who is responsible for providing medical management to patients through an interdisciplinary model, serving as the lead member of a care team consisting of a dietitian, exercise trainer, and licensed clinical social worker. The Nurse Practitioner will monitor and manage with appropriate interventions, obesity and the chronic comorbidities often associated.
