Referral Coordinator

Are you a highly organized and detail-oriented individual with excellent communication skills? Are you passionate about providing exceptional care coordination services? If so, we have an exciting opportunity for you to join our team as a Care Coordinator!

Position Summary:

As a Care Coordinator, your primary responsibility will be managing the relationships between employers, adjusters, nurse case managers, attorneys, funding companies, and our providers. You will play a crucial role in ensuring that patients are well-informed about their care plans and remain compliant throughout their treatment.

Key Responsibilities:

- Handle incoming and outgoing referrals for state, federal workers' compensation, and motor vehicle accident cases.

- Request prior authorizations for state workers' compensation cases.

- Submit certified liens for motor vehicle accident cases.

- Input demographic and payer information accurately.

- Screen and intake patients for federal workers' compensation and motor vehicle accident cases.

- Perform initial intake for state workers' compensation cases.

- Send prior authorization requests for federal workers' compensation to off-site approval, billing, coding, and collection departments.

- Draft letters for federal workers' compensation cases.

- Manage all paperwork related to state workers' compensation, federal workers' compensation, and motor vehicle accident cases.

- Track referral sources and statistics on monthly referrals, and arrange meetings with ownership and referral sources to expand the relationship with Core Medical Centers.

- Navigate insurance websites and ECOMP sites to upload necessary documentation for record submission and other case-specific paperwork.

- Utilize the specific systems used by CMC to track cases and key performance indicators (KPIs).

- Submit records within 24 hours of completed visits.

- Maintain constant communication with employers, adjusters, and nurse case managers regarding the status and progress of injured workers.

- Foster and nurture relationships between employers, adjusters, nurse case managers, attorneys, and CMC/ownership.

- Handle intake for all workers' compensation and motor vehicle accident exams, inputting them into the EHR system along with provider orders, plans of care, and CPT codes and ICD-10 codes as instructed.

- Schedule and participate in meet-ups with ownership and employers, adjusters, nurse case managers, attorneys, and funding company leadership to strengthen relationships with referral sources.

- Communicate and respond to funding companies, submitting bids for care based on providers' written care plans and fee schedules.

- Seek out networks and request contracts to join additional workers' compensation networks or set up meetings for ownership to negotiate contracts.

Performance Objectives:

- Timely submission and response to all referrals, employers, adjusters, nurse case managers, and attorneys.

- Create a respectful and caring environment for patients while adhering to best practices and balancing directives from referral sources.

- Follow Core Medical Center's Standard of Care Guidelines and report any staff or providers not adhering to these guidelines.

- Ensure patients are scheduled for all necessary visits, make-up appointments, and prior authorizations, allowing for efficient delivery of care.

- Complete each case, submit all required documents to the appropriate sources promptly, and follow up on any loose ends before closing the case.
