Core Medical Center is Hiring a Patient Care Coordinator Near Springs, MO

Position Summary:

The Case Manager is responsible for building and maintaining patient relationships throughout their care at Core Medical Center. This position is responsible for acquiring long term, quality patients for the clinic through the implementation of lead conversion systems. You will participate in educating the patients with the findings of an orthopedic exam, diagnostic imaging and delivering a financial breakdown of the overall Care Plan. This position will focus on understanding patient needs and wants, helping them overcome obstacles and empathizing with their struggles and challenges while influencing them to do what is in their best interest to improve their quality of life.

Performance Objectives:

  • Develop emotional connection with patients to understand their expected goals and desired outcomes
  • Perform an initial consultation on new patients to determine medical history, motivating factor and functional goals
  • Create and deliver financial estimates and long-term scheduling to patients based on Provider’s orders
  • Provide and ensure completion of all patient paperwork
  • Ensure that all of the patient’s needs are met, nothing gets overlooked, and care is consistent with the patient’s expressed desires
  • Execute patient follow up calls
  • Encourage, resolve concerns, and gather testimonials during care plan
  • Track and ensure patients complete care plans and are fully scheduled for future care
  • offer complementary services and determine benefits and cost for to present to patients.
  • Developed payment plans to ease patients ability to participate in care plan
  • Follow up on missed appointments and patients that did not start care
  • determine costs of reoccurring encounters and ensure patients are aware and scheduled for service and payment is collected or plan in place.

  • Exceptional interpersonal skills to quickly and effectively connect with patients

  • Advocate for the patient and motivate them to reach their health goals
  • Understand, align, and communicate our mission, vision, and values
  • Ability to define and take massive action to accomplish your own personal, professional and financial goals
  • Be comfortable discussing finances and asking for payment.

Education & Experience:

  • High school diploma or equivalent
  • 2 years of sales or management experience
  • Having some medical experience (Not Required but preferred)

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer and using a phone

Hours expected to work are 9-1, 3-7 MWTH, 1-7 T, 9-2 Fri. Starting pay is $16-18 a hour and after 90 day a chance for a raise, health insurance. After 1 year simple IRA with 3% match
