Description

The Job Coach assists employee/members with disabilities to learn and accurately carry out job duties in their employment at COPE EVOLVE. The job coach provides one-on-one training tailored to the needs of the employee/member; develops a specific plan as to how to best train the employee/member to work more and more on his/her own until completely self-sufficient and able to perform job duties accurately and effectively without assistance. The job coach supports the employee/member through advocacy, disability awareness-building, job adaptations, social support, problem-solving, and the development of natural supports to allow the job coach to phase out of direct involvement. Serves as a participating member of the employee/member’s treatment team.

Hours: Monday - Friday 8:00am - 5:00pm

Essential Duties & Responsibilities:

Job Coaching:

  • Assist the employee/client in performing the work responsibilities in such a way that fosters independence.
  • Continually assess the vocational, social, communication and independent skills required by the specific job setting.
  • Perform task analysis of each task as needed and provide guidance to employee/client.
  • Plan and schedule work assignments, train/supervise employee/clients on job procedures, use of equipment, and safety, and advise, check and assist in correcting errors.
  • Facilitate relationships between employee/client and supervisor and co-workers.
  • Identify, develop, and implement positive behavior support strategies.
  • Monitor and document employee/client progress, participation, and behaviors in activities, training and work time.
  • Identify job site modifications as needed. Recommendations are to be discussed with the Employment Services Team Leader first and Human Resources.
  • Comply with the policies and procedures of the job sites.
  • Report to the Employment Services Team Leader any needs, problems or concerns.
  • On a regular basis, report to the case manager.

Minimum Education/Experience Qualifications:

  • Bachelors or master’s degree, no experience necessary or
  • Associate degree plus two (2) years behavioral health work experience or
  • High School Diploma or GED plus Peer Support Specialist certification plus three (3) years of behavioral health work experience or
  • High School Diploma or GED plus Medical Assistant certification plus three (3) years of behavioral health work experience or
  • High School Diploma or GED plus four (4) years of behavioral health work experience

Preferred Qualifications:

  • Bachelor’s or Master’s degree in a field such as Psychology, Social Work, Human Services, or Rehabilitation and work experience in vocational services.

ADDITIONAL REQUIREMENTS:

Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record. An unacceptable driving record includes (but is not limited to) the following:

  • more than two [2] moving violations in the last three [3] years or;
  • more than one [1] at-fault accident within the last three [3] years or;
  • any major violation (i.e. DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years.

Other driving related violations may be considered unacceptable. At time of job offer, submission of 36 month driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.

Applicants must be able to pass a pre-employment drug screen.
