Job Description

We are looking for a Registered Nurse for a Full-Time Home Care Position to care for our patients and facilitate their speedy recovery in Queens. You will be responsible for educating patients and their families on prevention and healthy habits. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. The goal is to promote patients’ well-being by providing high-quality nursing care.

Responsibilities:

• Report to the Clinical Manager on a daily basis
• Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges
• Develop the Plan of Care in coordination with all health care professionals assigned to the case
• Provide documentation using the provided iPad within the required time frame
• Be knowledgeable in the use of the current OASIS assessment for all patient admissions, re-certification, transfers and discharges
• Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan

Qualifications:

• Licensed Registered Nurse in the state of New York with at least 1 year of clinical experience; Home care preferred
• COVID Vaccinated
• Oasis experience strongly desired
• Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus
• Have reliable transportation and be willing to commute to provide the best in-home patient care.
• Willing to take advantage of our excellent training system comprised of in-house and field orientation and become an elite healthcare provider!
• Some weekend days are required, and summer and winter holiday commitments as needed.
