Connecticut Community Care currently has several openings for Care Managers to join our growing organization. These positions are based in Connecticut and require travel to various locations.

YOUR ROLE
The Care Manager assists older adults and people with disabilities to obtain the services and supports that enhance their wellbeing and to live at home with dignity and respect. The care manager provides person-centered assessments that identify goals, needs and preferences and form the foundation for individualized care plans. Ongoing care management includes referrals to providers and linkages to benefits and resources and monitoring of these tailored service plans by telephone, videoconferencing and in-home visits.
Performs all duties in a manner that fosters the achievement of the organizations mission to identify choices and provide services to help people of all ages, abilities, and incomes to live at home.
REQUIREMENTS
· Reliable transportation, valid driver’s license and current automobile insurance.
· Willingness to travel regularly to client’s homes or nursing facilities within designated Connecticut region.
· Successful candidates are required to be fully vaccinated for Covid-19, absent medical, religious, or pregnancy related accommodations and will be required to submit proof at time of hire.
YOUR EXPERTISE
· A Bachelor’s degree in nursing, health, social work, gerontology or fields related to care management (counseling, mental health, psychology, gerontology, sociology, RN (licensed in the State of CT), rehabilitation, public health, or human services.
· Minimum of two years’ experience in health care or human services (including but not limited to community, hospital, institution or behavioral health).
· Knowledge of home and community based service systems preferred and understanding of psychological, human development, social, health, and economic factors influencing the attitudes and behavior of individuals and families.
· Care Plan Development - Demonstrated skills in person-centered approach. Ability to establish and maintaining supportive relationships.
· Knowledge of community resources available to individuals and families; an ability to mobilize resources into a coordinated and comprehensive plan of care.
· Familiarity with funding and financial sources; Medicaid, Medicare, managed Medicare, private insurance, pooled trusts, recoupment, cost share and applied income.
· Assessments - Experience in conducting comprehensive, systematic, person –centered assessments in community settings, homes, hospitals and nursing homes.
· Strong advocacy and communication skills.
· Strong understanding of the philosophy and importance of person centeredness, self direction/self determination and independent living.
· Experience building rapport and relationships with individuals, families and community resources.
· Experienced with interviewing, observation and analysis.
· Strong written, verbal, computer and skills necessary. Microsoft.
