About you:

ConcertoPACE of Los Angeles is seeking a passionate Certified Nursing Assistant for the Program of All-Inclusive Care for the Elderly. The Certified Nursing Assistant will provide safe delivery of personal care services to PACE participants in their home and at the center. They will work at the PACE center to assist participants in activities of daily living (ADLs) while in the center, providing companionship, and monitoring for safety.

Responsibilities:

• Cares for PACE participants by assisting with activities of daily living at the center. Escorts participants to any offsite medical appointments, as assigned. Monitors participant’s safety on or offsite. Assists participants at the center to ensure participants’ needs are met.
• Facilitates the movement of PACE participants between service appointments in the PACE Center.
• Assists the day center staff with meal service during snack and lunch periods.
• Assists the day center staff to escort the participants to the clinic, rehab, and other activities.
• Assists the PACE driver/dispatcher with unloading/loading participants safely from the bus through door and back on to the bus utilizing all safety training, safety equipment and skills.
• Provides translation services as needed, if qualified.
• Cleans participant areas in the day center between meals and activities.
• Assists the home care team as needed.
• Other duties as requested/assigned.

Requirements

• High school diploma or equivalent is preferred.
• Certified Nursing Assistant
• One year of experience with a frail, disabled or elderly population is required.
• Operates in full compliance with all applicable federal, state, and local fire, health, safety, sanitation, Emergency procedures and other Standards prescribed in law or regulations.
• Capable of and experienced in assisting individuals with special mobility needs.
• Knowledge of principals, practices, standards, and techniques of position.
• Knowledge of how to use and maintain positions equipment (vehicle, lift, tie-downs).
• Ability to lift/carry up to 25 pounds.
• Comply with ConcertoCare's COVID-19 Vaccination policy

Base Salary/ Wage Range $20 to $22 an hour. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown. COVID-19 Vaccination Policy

Our COVID-19 Vaccination Policy is aligned with the CMS Omnibus COVID-19 Health Care Staff Vaccination Rule that mandates providers who work in Medicare and Medicaid certified facilities to be fully vaccinated to best care for our frail and elderly populations. Based on this CMS requirement, ConcertoCare requires all “frontline workers” to be fully vaccinated.
