Companions and Homemakers, a leader in the home care industry in CT, is currently seeking Candidates to fill a Field Coordinator position in the Stratford Area. The selected candidate will enjoy a 40 hour work week over 5 days. 

Responsibilities: To ensure that clients are being taken care of to the standards set by C&H in order to promote home care as a viable and desirable alternative to institutional care. To observe and report information about clients' needs and caregiver performance in order to help us match the best possible caregivers to our clients.

Overview of Required Skills/Traits: Compassion for the elderly, straight forward communication skills, common sense, polite and professional phone manner and a dedication to the company vision, observational skills, strong verbal ans written communication skills, time management skills, attention to detail, and ability to be a “private detective” for evaluating caregiver performance, ability to adapt to last minute schedule changes..

Duties:

• Fill in for clients with last minute openings, fulfilling the duties of a companion, homemaker, or personal care assistant depending on the clients’ needs
• Represent the company in a positive and professional manner
• Provide temporary care for new clients until a suitable ongoing match has been made
• Act as a liaison with the management/staff of facilities where our clients reside, and communicate feedback obtained from these visits
• Perform home visits for all new private clients to observe services and ascertain whether the expected quality of care is provided
• Escalate any concerns immediately to direct supervisor pertaining to observation or learned information of clients, caregivers or office staff
• Follow weekly schedule of visits to check on caregiver attendance, job performance and client well-being based on direction of supervisor
• Periodically transport caregivers to/from client's residence
• Perform spot checks via phone weekly to ensure caregivers are arriving on time to their clients and staying their full shifts as we show them to be scheduled.
• Develop a working knowledge of company policies and procedures, services provided, and overall philosophy of the company. Use all available training tools for this purpose including training documents, company websites, or one on one training
• Carry a company cell phone so that you may be reached for any emergencies where a fill in caregiver is needed for a client. Check voicemail messages regularly and in a timely fashion.
• Make every effort to be reliable during inclement weather. The job of a Field Coordinator is to be available during emergency situations and to fill in when urgent services are needed in order to meet the care of a client. This makes it necessary that he/she is reliable, on time, and expected to overcome obstacles in order to be available during inclement weather.
