Pay Rate: $21.27/Hr

Primary Responsibilities And Duties
- Meets with Community Living, Inc. client to determine skills and job fit.
- Is responsible for collecting and compiling and organizing the participant’s individual record.
- Coordinates individual planning process.
- Completes clear and concise paperwork.
- Maintains/schedules caseload to ensure maximum efficiency and the quality of service.
- Administers interests inventory and functional testing.
- Provides transportation to job site for job evaluation.
- Documents skills, work needs, and accommodations onsite for the participant.
- Interviews family and team members to compile pertinent information.
- Completes all intake paperwork.
- Attends all pre-service staffings.
- Develops additional employment sites.
- Maintains the assessment site manual in accordance with CARF standards.
- Makes final determination and recommendation for employment.
- Conducts all meetings with all involved team members.
- Must maintain and adhere to all CARF and funding guidelines.
- Completes and maintains all required trainings.
- Performs other incidental and related duties as required and assigned.

Education, Experience, Skills:
- High school diploma or GED required. Bachelor’s degree in social service or a related field preferred.
- One year of experience working with individuals with intellectual and/or developmental disabilities is preferred.
- Possess sound oral and written communication skills as well as decision-making skills.
- Possess good listening and comprehension skills along with strong leadership skills.
- Must be able to write and document in clear, concise professional manner.
- Ability to work as a team member.
- Must be adept at balancing multiple caseloads.
- Demonstrate mastery of skills required for all programmatic aspects of ES.
- Possess the ability to use Microsoft Office applications including, but not limited to Outlook, Word, and Excel.
- Driver’s License
- Environment may vary from indoor to outdoor work as well as working in a wide temperature range. May have to operate light machinery such as floor buffers and pallet jacks.
- Physically assist participants as needed.
- Must adhere to the dress code of various employers.
- Moderate Work – Exerts up to 50lbs of force occasionally and 20lbs frequently. Standing, sitting, reaching, typing, and walking are common.
- Variable work schedule. May vary based on participant and employer needs. May include evenings, weekends and split-shifts.
