This position requires that the person has a successful verification and completion of the employee’s reference checks, education, employment experience, licenses, certifications, necessary background checks and other screening procedures used to assess the applicant’s good moral character and overall suitability to be employed for this position. · Must have successful training and performance of the following: personal care, safety, medication management, equipment, housekeeping, and psychological/sociological issues.
· Assist with the supervision of clients and provide support as outlined in their plan of service.

· Administer personal care as described in the plan of care when necessary.
· Assist the individual in attempting/completing their goals and objectives as outlined in their plan(s).
· Prepare and maintain records of client progress and services performed.
· Report changes in client condition to the direct supervisor.
· Perform housekeeping duties, such as cooking, cleaning, washing clothes or dishes, or running errands.
· Perform healthcare-related tasks, such as monitoring vital signs, medication, and using medical equipment.
· Prepare nutritious meals and follow menu as provided.
· Provide opportunities in the community for individuals such as social groups and individual activities when the contract requires it.
· Transport clients to locations outside the home for appointments, errands, and leisure.
· Provide valuable client information when necessary and when requested to client care team members.
· Provides emergency support (CPR & First Aid) when necessary.
· Communicate with fellow employees, supervision, and administration when necessary.
· Complete job duties in an organized, planned, and prioritized manner.
· Regularly inspect equipment, structures, or materials to ensure there are no issues. Report accordingly if issues are noticed.
· Review paperwork and training materials that are pertinent to the role’s success.
· Document and report as required by the individual’s plan of service and CHHS policies and procedures.
· Promote positivity and productivity.
· Attend required meetings, in-services, and training.
· Complete tasks and other duties as assigned by supervision and administration.· Be understanding of individuality; differences in ability, personality, motivation, learning, and interests.
· Maintain cultural awareness and sensitivity regarding diverse cultures, beliefs, religions and psychological development.
· Have the ability to teach others in a positive fashion.
· Have effective information gathering and problems solving skills.
· Ability to maintain records and compose correspondence related to the work.
· Ability to legally transport and accompany individuals to appointments away from residence.
· Ability to handle the physical demands of the work, including: lifting and positioning individuals, walking, escorting, and adverse behavior management practices.
· Ability to communicate effectively with others.
· Ability to effectively and appropriately work with clients who have various cognitive and physical disabilities.
· Ability to establish and maintain effective working relationships with individuals and groups.
Ensure that team members do not lift any individual, without assistance, weighing in excess of fifty pounds, except in emergency situations (e.g., fire evacuation and lifting a person). Must be able to lift up to 80 lbs. on a rare‑basis from approximately ground to waist level; ability to lift 26 to 75 lbs. on an occasional basis, ability to lift 10 to 25 lbs. on a frequent basis and lifting under 10 lbs. on a frequent basis. The carrying demands are the same as the lifting demands in terms of weight carried.
