Registered Nurse, Home Health
Community Home Health Care of West Michigan is looking for compassionate and caring Registered Nurses to join our home health team of providers in Muskegon, MI, and surrounding areas. Our home health nurses enjoy the one-on-one patient care they provide to our home bound patients. We need full-time nurses but if you would like to pick up home visits a few times each week, we would like to talk with you, too. Apply Now!
This position is available full-time, part-time, and PRN. Whether you want to visit patients daily or schedule them around family events or other obligations, home health care offers that flexibility. Our team appreciates the flexible schedule that home health offers.
We provide care to home bound patients in Muskegon, Ottawa, and Oceana counties.
Requirements:
- Graduate of nursing school and currently holding a RN license.
- 1 year of previous nursing experience, preferred
- Previous home health care experience, preferred
- Ability to communicate effectively and work as a part of an interdisciplinary team
- Strong ability to use critical thinking and deduction when performing job tasks and evaluating patient needs
- Ability to consistently connect with patients one-on-one
- Responsible, compassionate, and caring demeanor
Job Responsibilities:
- Providing services in accordance with the patient’s Plan of Care
- Performing initial evaluations, building Plan of Care, and regularly reevaluating patient’s nursing needs
- Initiating appropriate preventative and rehabilitative nursing procedures
- Preparing clinical and progress notes for each patient visit via EMR
- Preparing summaries of care conferences on patients in a timely manner
- Informing Physician and Director of Clinical services of changes to patient’s condition and/or needs as appropriate
- Counseling the patient and family/significant others in meeting nursing and related needs
- Supervising and teaching other nursing personnel and CNAs/HHAs
- Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition.

Pay: $35.00 - $85.00 per visit (Compensation is based on experience)
