SUMMARY: Primary functions include: planning, providing, and evaluating skilled nursing care to hospice patients and families in their home. The ideal candidate will evaluate and coordinates their hospice course of care. Confers and collaborate with the physician and other team members in implementing the care plan.
DUTIES AND RESPONSIBILITIES:
  • Makes the initial visit, to evaluate the patient and family needs. Develops and implements nursing treatment plans for all patients referred to him/her, based upon data from the nursing history patient and family and assessment and the physician’s plan of treatment.
  • Provides services requiring substantial and specialized nursing skill. Initiates appropriate preventive and rehabilitative nursing procedures, coordinating plan of treatment with physical, speech and occupational therapists, medical social worker, and home care aide as necessary.
  • Regularly re-evaluates the patient and family needs, and revises the plan of care as appropriate.
  • Plans for and participates in Hospice conferences with the Hospice care team.
  • Reports significant changes in patient’s condition, signs, symptoms, and needs to the physician and other agency personnel. Consults with the Hospice Medical Director as necessary.
  • Prepares concise legible clinical notes, progress notes, summary reports, re-certifications, and discharge summaries completely and in a timely manner.
  • Makes verbal and written reports to the physician as required by agency policy.
  • Is responsible for tasks delegated to the Hospice Care Aide and the manner in which they are accomplished. Performs supervisory visits per agency standard on home care aides.
  • Teaches, supervises, and counsels the patient and family to meet patient and family needs identified in the plan of treatment.
  • Participates in discharge planning when appropriate.
  • Maintains coordination with all other disciplines as needed for patient care.
  • Assists with patient care audit and clinical record reviews.
  • Provides nursing support to patient and families as per hospice regulations.
  • Reports regularly to the Hospice Medical Director regarding patients, staffing, and other information relevant to the needs of the patients.
  • Participates in the on-call schedule as shared by agency nurses.
  • May on occasion work in the inpatient Hospice Care Center.
  • Utilizes appropriate communication lines to problem solve personal and departmental issues.
  • Accepts other duties as assigned, which may vary based on agency needs.

Qualifications
  • RN required license for Washington and Oregon.
  • Prefer one year acute care setting with IV experience.
  • TB screening.
  • Proof of COVID 19 Vaccination or bona fide medical/religious exemption.
  • Current CPR certification.
  • Valid driver’s license, reliable auto, and proof of auto insurance.
