SUMMARY:

The Primary Care Physician will be responsible for providing complete clinical care for patients of the Community First Medical Group. The PCP will be integral in referring patients requiring emergency care to our Emergency Room, coordinating with our Hospitalists during inpatient stays, and providing post-acute and ongoing patient care services upon inpatient discharge. The PCP works in conjunction with the Community First Medical Group in implementing and maintaining quality care programs.

Primary Duties and Responsibilities:

• Obtain medical, social and developmental histories

• Perform physical exams and immunizations on all patients

• Initiate laboratory exams and studies as indicated

• Provide health education and counseling

• Management of acute and chronic illnesses to and above community standards 

• Consult with and Refer to appropriate specialist for high risk or complex patients

• Exercise judgment in establishing priorities of medical care in triage situations

• Assure accurate and timely completion of all clinical and professional records and documentation in compliance with local, state and federal regulatory, licensure and accreditation requirements

• Participation in Quality Improvement programs

• Provide case management with other health care team members

• Provide consultations to other Physicians, Nurse Practitioners, Physician Assistants, Medical Assistant

Requirements

Required Education/Experience and/or Licensure/Certifications:

• Active Illinois Medical License (M.D. or D.O) J1 waiver and H1 encouraged to apply as well.

• Board Eligible/Board Certified in Family Practice/Internal Medicine

• Excellent Customer Service (Bedside Manners)

• Bilingual English/Spanish preferred

• Training experiences as mandated by the American Board of Family Medicine for board eligibility are required.
