About Valley View Health Center…

Valley View Health Centers is devoted to supporting the mission of The Community Action Committee of Pike County to improving the well-being and independence of those we serve; by “providing access to affordable, high-quality, integrated health care for all.” We deliver adult and pediatric primary care, behavioral health, dental, and vision services throughout the Appalachian region of Southern Ohio. We operate seven center locations located in Adams, Jackson, Pike, and Scioto counties, focused on growing Healthy Communities.

Position Impact and Purpose…

Our centers are dedicated to growing healthy communities by providing access to affordable, high quality, integrated health care for all. As a primary care nurse, you will serve in one of the many integral roles of a diverse patient care team. The primary care nurse is responsible for ensuring a positive patient experience; through providing excellent patient care and effective communication in an office setting. Purpose of the primary care nurse position is to positively impact patient’s well-being in collaboration with the health care team.

Morning Huddle

  • Prepare Pre-Visit Planning (PVP) for assigned provider and primary care team
  • Review and ensure PVP is completed each morning
  • Arrives timely, pleasant demeanor, positive attitude, and team oriented

  • Answer live patient calls for assigned Primary Care Team as assigned by lead nurse or Clinical Services Manager
  • Provider excellent customer service/communication
  • Collaborate with team and provider to answer and respond to patient questions/requests

Medication Prior Authorizations (as assigned)

  • Complete Prior Authorizations for assigned Primary Care Team as assigned by lead nurse or Clinical Services Manager
  • Follow up with patients regarding status of Prior Authorizations
  • Attends meeting, conferences, and/or trainings

  • Will provide excellent direct patient care
  • Will complete clinic workflows as designed
  • Collaborate with assigned provider as necessary to ensure quality patient care
  • Complete daily tasks as assigned
  • Provide immunizations as ordered
  • Complete patient point of care testing as ordered
  • Complete all EHR documentation in an accurate and timely manner
  • Will maintain a safe work environment and adhere to all compliance and safety policies and procedures

Working Conditions…

Primary work conditions are indoors in climate-controlled setting in an office like environment. The employee is regularly to sit; use hand to finger, handle or feel objects, tools or controls; reach with hands and arms; and talk and hear. The employee is occasionally required to stand, walk, stoop, kneel, crouch or crawl. The employee must regularly lift and /or move up to 10 pounds and occasionally lift and/or move up to 25 pounds
