Triage Nurse needed for a contract opportunity with Yoh’s client located in Davis, CA!
Top Skills You Should Possess:

• Experience in Community Care or other Outpatient setting

What You’ll Be Doing:

• The Triage RN is responsible for telephone triage and advice services for patients and community partners (such as hospitals, urgent care centers, pharmacies, laboratories, diagnostic imaging centers, and other health care offices)
• Answer the nurse triage line, which may include, but is not limited to, performing an assessment of patient symptoms, and referring them to the appropriate level of care (home care, appointment with primary care provider (PCP), and/or escalating treatment to higher level of care like the emergency department)
• Collaborate with case management, nursing teams, providers, and other departments to ensure quality patient care
• Conduct telephone triage following established procedures and protocols, and document calls as required
• Schedule is Monday – Friday, 8am – 5pm; RN will work from home 4 days/week and will be in-clinic 1 day/week

What You Need to Bring to the Table:

• Active and unrestricted California RN license
• Current BLS certification
• Bachelors Degree in Nursing preferred
• Minimum of two (2) years of experience in direct patient care
• Experience in a community healthcare center preferred; outpatient primary care setting experience preferred
• Training in motivational interviewing preferred
• Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred
• Strong interpersonal and customer service skills required
• Strong computer and data entry skills required; electronic health record system usage required
• Strong analytical and problem-solving skills required
