Columbia County Senior Services is currently hiring a full-time Case Manager based out of our Lifestyle Enrichment Center in Lake City, FL. Responsibilities include in home and telephone assessment of seniors using standardized instruments, development of care plans, arrangement of needed services, and referral to outside agencies as needed. Successful candidates will have great attention to detail and be able to work independently on multiple projects. Experience in case management and aging services preferred. Bachelor's degree in social work, sociology, psychology, nursing, gerontology or a related social services field preferred. Combination of education and related experience may be substituted for bachelor's degree. Candidates must possess a Florida driver's license, a clean driving record, and be able to pass a Level II criminal background check.
