Position Summary: Medical Transcriptionist transcribes reports documented/created by physicians and other healthcare practitioners. The types of documents include items such as, letters, chart notes and reports. Ability to work from home. The Medical Transcriptionist transcribes written documents into electronic health record visits for patient visits. These written documents created by physicians or other healthcare professionals, become part of permanent written records by way of your transcription services.
Reports to Practice Administrator
Responsibilities:
Transcribing dictation for a variety of reports, including:
o Patient histories
o Physical examinations
o Emergency room visits
o Surgeries
o Chart reviews
o Consultations
o Discharge summaries
Translating medical jargon and abbreviations into their expanded forms.
Ensuring the accuracy of patient and health care facility records.
Editing as necessary and returning reports in electronic form for review.
Communicating with Physicians and administrative staff to fact-check information and medical terms, typing a variety of medical documents including patient reports or public health advice and ensuring they complete writing tasks within established deadlines for their employer.
Transcription and coding of patient information into EHR system including subjective notes, labs, assessments, plans and coding from handwritten visit notes of the attending physician.
Service level of two-day turnaround maintained on all visit notes for a 30 patient per day physician schedule.
Enter prescriptions and perform control audit reviews of prescriptions entered by the rest of the office staff.
Reprogrammed in ICD-10 codes where there were ICD-9 codes
Maintain safe, secure and healthy work environment by establishing and following standards and procedures and complying with legal regulations.
Update job knowledge by participating in educational opportunities and reading professional publications.
Enhance practice reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to job accomplishments.
Generate revenue by recording billing information of services rendered, completing insurance forms and responding to insurance and other third-party inquiries.
Serve and protect the physician or healthcare provider practice by adhering to professional standards, policies and procedures, federal, state and local requirements.
Perform other duties as assigned or requested.
