Description

This position works closely with Liaisons and physicians to schedule, facilitate and maintain quality care for Sub-acute Trauma patients. This position also works closely with patients to ensure planning and continuity of care.

Job Duties and Responsibilities

• Collaborates with the physician and patients to schedule and coordinate care
• Educates liaisons and continuously communicates between physician and patients
• Anticipates, prevents, and manages emergent situations and communicates appropriately while documenting all patient communication & appointments in Sub-acute Trauma Program health systems.
• Supports and follows the mission, vision, values, and policies of Sub-acute Trauma Program and communicates regularly with the appropriate leadership and staff.
• Coordinates care plan that is appropriate, timely, effective, efficient, equitable, and patient centered
• Handles case management assignments and assists with making appointments on time and sending reminder & text reminders
• Facilitates multiple care scheduling with the local patients. Provides itineraries for out-of-town patients with outpatient and inpatient appoints
• Advocacy for options and services to meet an individual and family's comprehensive health needs and concerns through effective communication
• Data entry into health systems and provides timely and accurate updates to physicians and liaisons assigned to firms and offices.
• Develops effective working relationships and cooperates with medical team and liaisons while utilizing effective communication systems to patients and referring sources

Qualifications

Education

• High school diploma/GED required. Some college preferred.

Knowledge, Skills and Abilities

• Strong assessment, organizational and problem-solving skills
• Must be adept with Microsoft Office
• Required Bilingual (Spanish) speaking
