CO-MHAR INC is Hiring a Program Specialist Near Philadelphia, PA

Description

COMHAR is a nonprofit community based organization. Our mission is to provide health and human services that empower individuals, families and communities to live healthier, self-determined lives. We are seeking for a Program Specialist to join our Developmental Intellectual Disabilities program in Philadelphia, PA.

  • Coordination and/or completion of initial, annual and revision consumer assessment for assigned caseload
  • Identifying in the assessment the consumer’s individualized support need requirement for direct supervision or times when the consumer may be unsupervised
  • Disseminating the assessment for the development of the ISP, ISP Annual Update and all ISP revisions to all ISP plan team members 30 calendar days prior to the meeting
  • Providing training to Direct Support Professionals on the consumer Support Plan if a medication is prescribed to treat symptoms of a diagnosed psychiatric illness prior to the Direct Support Professional working with the consumer
  • Participating in the development of the ISP, ISP Annual Updates, and all ISP revisions
  • Attending the ISP, ISP Annual Updates, and all ISP revisions
  • Fulfilling the role of Plan Lead when a Supports Coordinator is not assigned to a consumer
  • Reviewing every 3 months (or more often if necessary) the ISP, ISP Annual Updates, and all ISP revisions
  • Reporting ISP content discrepancies to the Supports Coordinator as applicable and Team members
  • Implementing the ISP as written
  • Supervising, monitoring and evaluating services and outcomes identified in the ISP
  • Recommending a revision (addition, deletion, modification) to the service or outcome in the ISP when needed
  • Reviewing, signing and dating the monthly documentation of an individual’s participation and progress toward outcomes
  • Reporting a change related to a consumer’s needs to the Supports Coordinator as applicable and to plan team members
  • Reviewing the ISP every 3 months with the consumer and documenting the review of the ISP as required by DPW regulations
  • Disseminating the 3 month plan review documentation and recommendations to the Supports Coordinator and plan team members within 30 days of the review
  • Informing and documenting plan team members of the option to decline the 3 month ISP review documentation (except Supports Coordinator /Plan Lead)
  • Recommending a revision (addition, deletion, modification) to the service or outcome in the ISP when needed
  • Coordinating the services provided to a consumer
  • Coordinating the training of Direct Support Professionals in the content of health and safety needs relevant to the consumers on your caseload
  • Developing and implementing component services that will lead to advancement in the component or to competitive employment
  • Assure all medication management, physical site, and trainings are in compliance with DPW 6400 regulations.
  • Adhere to HIRS (Health Information Record Systems) standards in completing Medical and Programmatic Books.
  • Monitor the EIM system to ensure that incidents are being resolved in accordance with applicable regulations
  • Maintain clients’ Individual Records to ensure that Program Books are current and accurate
  • Support the development and maintenance of the behavioral component of the Individual Support Plan, including compliance with Human Rights Team process.

Requirements

  • BA 2 years of MH/MR experience, or AA 60 college credits in Human Service discipline 4 years of MH/MR experience.
  • Must be proficient in 6400 regulations.
  • Must be proficient in Microsoft Office Programs (Word, Excel, etc).
  • Supervisory experience preferred.
  • Valid driver’s license & own vehicle required.
