SUMMARY/OBJECTIVE:

This position is responsible for assisting clients to develop and implement their personal wellness program. Provides support and education related to physical fitness, nutrition and wellness. Encourages by example. Assists in training of volunteer mentors as needed. Provides group activity instruction. Works independently onsite and in community settings with individual consumers or leading group activities.

Essential Functions (Job Duties)

1. Provides community-based support services to ensure implementation of clients' health goals.
2. Manages a caseload of clients to establish and implement individual health and wellness plans.
3. Works collaboratively with clients' program staff to design and support individual member plans.
4. Provides fitness training, monitoring and continuing reassessment of exercise, nutrition and healthcare
5. Facilitates peer relationships among participants.
6. Actively participates in activities with member when support is needed.
7. Helps members maintain focus on personal goals.
8. Encourages member participation and movement along the continuum of wellness related lifestyle
9. Administers fitness assessments as needed and records data.
10. Work with clients, volunteers and community partners to increase client independence to work toward discharge

1. Assists in facilitation of team building activities.
2. Provides instruction and facilitation for group activities.
3. Works with clients to identify and establish a wellness support system outside of health and wellness program.
4. Assist in training new mentors, interns and volunteer mentors.
5. Assists in the supervisor of volunteer mentors.
6. Contribute to creation of manuals, videos and web based training materials.
7. Attend and/or participate in relevant trainings, webinars and conferences.

Required Education and Experience

1. Degree and/or experience in Health Science, Social Work, Rehabilitation, Psychology, Education or related field.
2. Personal trainer certification preferred
3. Required to maintain proper certifications as needed
