Social Worker
Citadel Healthcare is currently seeking the services of a compassionate social worker for the geriatric population to join our team.
Social Worker Essential Job Functions:
- Identifies the social and emotional needs of individual residents.
- Assists in arranging services to meet the needs of residents.
- Participates in resident care planning.
- Ensures the social and emotional needs or reach resident are met.
- Provides advocacy between the resident, family, facility staff and the community.
Education and Experience Requirements:
- MSW, LCSW, or LSW preferred
- Previous experience in long-term care facility is preferred
- Working knowledge of Medicare and Medicaid is preferred
- Experience in creating quality assurance/quality improvement initiatives
- Strong communication and multi-tasking skills
- Compassionate and empathetic personality
